AGREEMENT TO FURNISH SUMMER FOOD SERVICE PROGRAM

WHEREAS the

School agrees to supply (unitized/non-unitized) meals (inclusive/exclusive) of milk and juice to Sponsor with and for the rates herein listed:

School Breakfast Program $______Each

Special Milk Program $______Each

After School Snack Program $______Each

Summer Food Service Program $______Each

It is further agreed that School, pursuant to the provisions of the SFSP (Summer Food Service Program), attached copy of which is part of this agreement, will assure that said meals meet the minimum meal pattern requirements as to components and portion sizes, and will maintain full and accurate records that the Sponsor will need to meet its responsibility. These records will contain prepared food amounts, daily meal counts and menu records.

These records must be reported to the Sponsor promptly at the end of the calendar month. School agrees also to retain records required under the preceding clause for a period of three years from the date of receipt of final payment under this agreement. The period of time may be longer than three years if an audit is in progress. Upon request, all records and accounts pertaining to the program will be made available to representatives of the Indiana Department of Education, US Department of Agriculture and the General Accounting Office for audit or administrative review at a reasonable time and place.
