I and the IEEE Communications Society are nominating you for IEEE's distinguished membership grade of Senior Member.
It is a membership grade approved and assigned by the IEEE Admission and Advancement Committee only for IEEE members with 10 years in the profession who have demonstrated at least 5 years of distinguished technical and professional performance. A completed SM nomination /application form and three Senior Member/Fellow references are required. Since you are being nominated for Senior Member, the number of Senior Member references required for you is reduced to only two.
I ask you to complete the Senior Member Application Form ( which is available in three formats: online, as a downloadable pdf file and as an electronic document. The electronic version allows you to complete the form and submit it as an email attachment. The Senior Member Reference Form ( is also available in the same three formats and must be submitted by your references. For your convenience, I have attached the electronic application and reference forms but feel free to use the method that best serves your purpose and assures delivery of your application in a timely manner. Submission of your resume or curriculum vitae is optional but highly recommended.
I ask you to complete the nomination information portion of the application filling in my name and number on the nominator line and the entity nominator name as listed below. This will assure that you need only two additional Senior Member references and not the normal three. If you have difficulty in identifying two additional Senior Member references to support your application, you may contact me for assistance. If you have the names of two Senior Members to serve as your references, you may attach the reference form provided here or refer them to in your letter requesting their support of your nomination for IEEE Senior Member grade.
Upon the approval of your nomination, an attractive plaque will be sent to you and an IEEE Senior Member voucher given to you for one free new IEEE Society membership worth up to $25.00. A letter of commendation from the IEEE President can also be sent to your employer upon your request.
