TOP OFFICIALS 3 EXERCISE ROLE PLAYER REGISTRATION FORM

ACTOR INFORMATION SHEET

Please read and understand the following points. This will ensure that your participation in this exercise will be safer and more enjoyable. If you have any questions, please contact the Warren County

Health Department at 908-689-6693.

1.  The evening may be long and tiring.

You need to be at the site by 6:00 PM, 5:00 PM for the meal, and you probably will not finish until after 9:00 PM. Please make sure you are in good physical condition. If you have any health concerns or medical conditions, please call Sheila Risley at 908-689-6693, prior to the exercise. (Health or medical concerns will not necessarily disqualify you from participating).

2.  All participants must sign a waiver, to participate.

If you are not 18 years old or older and are not in the military, you may not participate in the exercise without parental permission. The waiver form (included) must be turned in prior to the exercise to Warren County Health Department to the attention of Sheila Risley.

3.  Dinner provided at 5:00 PM.

Exercise officials are planning a dinner before the exercise for all participants. If you are able to join us for dinner, please arrive no later than 5:00 PM. An official briefing will be held and the exercise will start promptly at 6:00 PM. Snacks and beverages will be provided during the exercise.

4.  Drive yourself to the exercise site.

Please drive yourself to the Pequest Trout Hatchery. (A map and directions are attached) You will be required to drive though a security check upon entering the Hatchery, so please allow extra time.

5.  Don’t be late!

The exercise will begin promptly at 6:00 PM, April 7th at the Pequest Trout Hatchery. Do not arrive late. You will probably be released by 9:00 PM; however, this is not guaranteed. You may finish earlier or later.

6.  Only bring a few items with you. There will be no place to keep them.

Bring your drivers license, keys, and a sense of humor. Do not bring cameras, valuable jewelry, large sums of money, unregistered friends or volunteers.

7.  Don’t overact.

It is very important to play your assigned role the best you can, but this does not mean you should overact. Overacting is unnecessary and can be dangerous for yourself and the emergency workers in the exercise. When you arrive at the exercise site, you will be assigned an injury or role. There will also be a briefing about your roles and what will happen during the exercise. If you do not know how to play you role or have questions about the briefing, please ask the volunteer coordinators who are there to help. If you are assigned the role of a psychologically distressed person, please act upset, not out-of-control.

8.  If you get hurt or have a real problem say, “This is a Real Emergency”

You must use the phrase “This is a real Emergency “ to tell exercise staff that you have a real problem and are not acting.

9.  You must check in and sign out.

When you arrive in the evening, you will sign in and be assigned an “injury,” A victim tag will be placed around your neck. This card must be turned in at the checkout station before you leave. Do not remove or allow anyone to remove this victim tag during the exercise. When the exercise is over, turn in your victim tag with the questions completed on the back.
