Job Summary

The Medical Records Clerk is responsible for the set up, filing and maintenance of patient clinical records and information. Answers the phone in a friendly courteous manner. Enters information daily into the billing system. Files a variety of documents on a daily basis.Works with and assists Payment Coordinator, Chief Finance Officer, and Medical/Clinical staff.

Essential Job Functions

  • Communicates phone calls in a timely manner.
  • Must have basic computer, 10 key, and typing skills, knowledge of Excel, Micro Soft, Word, Scanning and e-mailing.
  • Enters new patient information, transfers, discharges, admissions, re-certifications, all medical and clinical staff notes, including Physical and Occupational Therapists. Enters supplies, and resumptions into the system on a daily basis for billing to be completed.
  • Receive and enter medical information into the system on a daily basis.
  • Files various documents accurately on a daily basis.
  • Receive, distribute, send and confirm faxes through out the day.
  • Types various correspondences as asked by department supervisor(s) and/or MedicalDirector, CFO, Payment Coordinator.
  • Maintains current directory listings such as essential telephone numbers.
  • Knowledge of office equipment operating and user maintenance procedures, including but not limited to phone system, photocopiers, fax machine, postage meter, etc.
  • Maintains strict confidentiality of all participant information.

Non-Essential Job Functions

  • Participates on task groups and committees, with supervisor approval.
  • Is familiar with facilities’ policies, procedures and operations.
  • Accepts assigned duties in a cooperative manner and performs all other related duties as assigned by Supervisor.
  • Establishes and maintains an organized, clean, and orderly work station and storage room area.
  • Comes to work in neat, clean attire and consistently presents a professional appearance.

Working Conditions / Physical Demands

Ability to sit and stand for extended periods of time. Ability to communicate verbally with an excellent comprehension of the English language. Ability to move about with reasonable accommodation, and be able to reach, turn, twist and touch. Ability to lift up to 10 pounds and move objects short distances. TheMedical Records Clerk is required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds. Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.

Knowledge, Skills and Abilities

  • 1-2 years’ customer service experience; prior experience in a senior care environment a plus.
  • Computer experience including Word and Excel.
  • Excellent written and oral communication skills.
  • Positive energetic attitude.
  • Employee must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.
  • Must have excellent communication skills, be able to cope with mental and emotional stress related to the position, function independently, have flexibility and personal integrity.

Education/Certification

Education:High School Graduate, Computer Tech Certificates a plus. It is necessary to have effective social interaction skills and communication skills. Must have good organizational skills and be able to perform a variety of tasks efficiently.
