Conestoga High School Permission Form for One Day Field Trips
My child, grade , homeroom , has my permission to participate in the field trip to the National Constitution Center in Philadelphia on Friday, December 13, 2013.
General description of trip:
Students have been learning about the US Constitution in their US Government classes. This field trip is designed to extend their knowledge about the federal government and American politics. Students will hear an introductory presentation at the center and then explore the exhibits at the center while completing an assignment. We will also have lunch at the Reading Terminal Market, which is located about six blocks from the center. Students will be expected to walk with their assigned chaperone/group to the market for lunch.
The students will be departing school at approximately 8:00 AM and returning at approximately 2:15 PM. The mode of transportation that will be used is school buses.
The cost to each student is $7.50 for admission to the Constitution Center. Money is due to Ms. Whittaker, Ms. Lefkowitz, or Mr. Goodman by Wed. Dec.4. Please make checks out to Conestoga High School. Students should also bring a lunch or money to purchase lunch.

In granting this permission, I understand that my child is required to abide by all school and Tredyffrin/Easttown School District codes of conduct as a condition for participation in this field trip. I have discussed this condition with my child and represent that she/he agrees to this condition. Tredyffrin/Easttown School District will not be responsible for monetary deposits lost due to the cancellation of this trip for any reason.
If my child is over the age of eighteen (18) years, he/she assents to the above and to abide by all Conestoga High School and Tredyffrin/Easttown School District codes of conduct as a condition for participation in this trip as witnessed by his/her signature.
