Waiver and Release

This WAIVER and Release is made by the undersigned to Ice Forum Hockey Association (IHA), a subsidiary of IceForum, Inc.

WHEREAS, IHA is organized for the promotion of the sport of Ice Hockey;

WHEREAS, IHA coordinates ice hockey activities at area ice skating rinks; and

WHEREAS, the undersigned desires to engage in activities, such as ice hockey games and practices, coordinated by IHA.

NOW, THEREFORE, for and in consideration of being permitted to voluntarily participate in IHA activities, and other good and valuable consideration, receipt and sufficiency which is hereby acknowledged, the undersigned hereby:

  1. Acknowledge that the undersigned is familiar with the rules of the IHA, Atlanta IceForum, IceForum at TownCenter and agrees to play and adhere by such rules.
  2. Acknowledges that the undersigned is familiar with the “zero abuse” policy and agrees to abide by the “zero abuse” policy at the ice skating rinks.
  3. Acknowledges that any physical or verbal abuse of game officials or referees before, during, or after any ice hockey game or function is against IHA rules and is punishable by ejection from such game and suspension from future activities of IHA
  4. Expressly consents to be governed by all rules currently existing or later enacted by IHA
  5. Acknowledges that ice hockey is a physical sport that might result in injury, and accepts, and assumes the risk of injury or damage to his/her person or property that might result from participating in IGA activities;

6. Remises, releases, agrees to indemnify and hold harmless, and forever discharges each IHA, IceForum at Town Center, their respective affiliated companies, firm and corporations, and the respective officers, directors, employees, agents and attorneys, assigns and predecessors in interest, from all actions, causes of action, suits, debts, dues, sums of money, accounts, covenants, contracts, controversies, agreements, promises, trespasses, damages, judgments, executions, claims and demands of whatsoever kind and nature at law and equity, which the undersigned, the undersigned heirs, executors, administrators, successors and assigns, ever had, now have or hereafter can, shall or may have, for upon, or by reason of any matter, cause or thing of including without limitation any matter arising from or in connection with the undersigns voluntary participation in IHA activities; and

7. Acknowledges and accepts that the IHA makes no guarantee as to team or division assignment.

This Waiver and Release may not be changed except by writing executed by both IHA and the undersigned.

The undersigned represents to IHA that the undersigned is over the age of eighteen (18) years old and has full power and authority to enter into this Waiver and Release.

This release is given voluntarily by the undersigned and is not based on any representations or statements of any kind made by anyone to his, her, or its representatives as to its legal merit, legal liability, value of the claims, or any matters relating thereto.
