(Clinical Evaluation of Respiratory Therapies)

Diagnostic Medical Testing’s C.E.R.T. program is a diagnostic monitoring program designed to evaluate in home therapies and patient compliance. The objectives of the program are:

·  Ensure compliance with prescribed cardiopulmonary therapies

·  Disease management and education to help patients with your individualized plan

·  Qualification of therapies per insurance guidelines (as needed)

·  Improve quality of life

The C.E.R.T. program will follow your patients in the home environment quarterly and PRN. It consists of the following:

·  Review of all prescribed cardiopulmonary therapies and orders to ensure patient compliance in the home environment.

·  Education

o  Review of the disease process and management (including lifestyle changes/modifications)

o  Treatment procedures

o  Prescriptions and medications

o  Home safety

o  Equipment checks

o  Available Cardiopulmonary resources.

·  Rest and exertion oximetry is completed at each visit by licensed clinical professionals and to insurance specific guidelines

·  Overnight oximetry as indicated per assessment

·  Patient assessment including heart rate, respiratory rate, and blood pressure

The C.E.R.T. program is another resource for you, as a physician, to ensure that your patient’s are being compliant in the home setting. This program provides licensed clinician’s thorough and detail oriented clinical evaluations directly communicated to you through email/fax at your preference.

Benefits of using Diagnostic Medical Testing (DMT) to complete the in home clinical evaluation for your patients:

·  When a clinical issue arises in the home (i.e. desaturations on exertion), the patient is already qualified per insurance guidelines once our testing is completed. This makes for a timelier set-up of the appropriate patient care and/or equipment, tailored for your specific patients.

·  There is a quicker line of communication to you, the physician, because we eliminate redundant steps within the qualification and testing process that usually involve a number of other parties.

·  DMT believes in quality of care vs. quantity of care – we will ensure that your patients have a complete understanding of their home respiratory therapies, medications, equipment, and any accompanying orders from you, the physician.
