Job Description for post of Pathways Coordinator
Purpose of role - to manage and co-ordinate the efficient and effective delivery of Breakthrough’s Pathways Project.
A) General duties and responsibilities
A1) at all times to comply with Breakthrough UK policies and procedures in your work and your conduct and behaviour at work.

A4) to provide regular and ad hoc reports to Senior Management as required.
A5) to contribute to the efficient and effective operation of the organisation and its business activities by working collaboratively and co-operatively with other staff at all times.
A6) to undertake any other duties as required, appropriate to the post.
A7) to participate pro-actively in regular supervision and annual performance appraisal as appraiser and appraise as required.
B) Project specific duties & responsibilities – Pathways Project
B1) to be responsible for the design, delivery and administration of the Pathways Employment & Independent Living Skills Project across the Greater Manchester area and to ensure compliance with funder (Lloyds Foundation) requirements and grant conditions and achievement of agreed outputs and outcomes.
B2) to establish and maintain relationships with other relevant organisations across Greater Manchester to promote the Pathways Project and to identify potential participants for referral to the Project.
B3) to create content and learning materials for, and to plan, organise and deliver, Employability and Independent Living Skills workshops for Pathways Project participants in order to achieve the desired learning outcomes.
B4) to ensure that the Project workshops meet the access and learning needs of participants by identifying and removing any disabling barriers.
B5) to source external facilitators or tutors to deliver workshops as appropriate
B6) to support Pathways Project participants to identify their individual goals and aspirations for employment applying a holistic and person-centred planning process.
B7) to support pathways Project participants to self-complete the Work or Independent Living Outcomes Star to enable them to measure their progress and attainment.
B8) to ensure the monitoring, measurement and evaluation of Project activity in order to provide evidence of its impact.
B9) to complete Risk Assessments for the Project and participants as necessary.
B10) to create and maintain Pathways Project participant records on our Lamplight CRM system in an accurate and timely manner.
B11) to support Senior Management and Fundraising Officers on the identification and preparation of tenders, bids and applications for new revenue streams to sustain and develop Project activities and other related business opportunities.
B12) to develop and implement a set of service quality standards and quality management procedures including associated documents and operating procedures for the Pathways Project and ensure the consistent application of these.
B13) to develop and implement a Referral pathways and ‘Customer Journey’ for the Pathways Project by relevant staff.
B14) Establish and maintain relationships with other service providers, employers and organisations in order to secure suitable vocational training, work placement, volunteering and employment opportunities for participants of the Pathways Project.
B15) Facilitate peer support groups for Pathways Project participants as required.
C) Staff and systems – duties and responsibilities
C1) Supervise volunteers and occasionally specified staff members as required including assessment of performance through observations, evaluation of records and 1 to 1 supervision meetings.
D) Other key duties and responsibilities
D1) Support the Operational Services Manager and other members of Senior Management in the management of Breakthrough services, projects and other business activities as required.
