The Derek Cordova Spirit Scholarship
Application for 2014-2015 School Year
The Derek Cordova Spirit Scholarship will be granted to one or more male or female John Mall High School Graduate(s). The intention of this scholarship is to help with the continuance of college education.
Criteria must be met in order to qualify for this scholarship.
The Qualifications are as follows:
Must have attended John Mall High School for the entire 4 years.
Must have maintained at least a 3.0 or better grade average.
Must have been actively involved with at least one of the following:
FBLA, Student Council, PRIDE, SADD, NHS, Year book, or Community Volunteer.
Must have been actively involved in one sport per high school year.
1. Application must be typed.
2. Attach a one-page self-description appendix.
3. Attach two letters of recommendation.
4. Attach a letter of admission from the college you plan to attend.
5. Attach copies of your high school and/or college transcripts that indicate your class rank, GPA, and ACT scores.
6. Attach a recent photograph of yourself.
All applications will be considered. Deadline for application is May 7, 2015 at 3:00 p.m. Deliver completed application to the John Mall High School Counselor.
