Reception Staff at Wistaria and Milford
Last year we welcomed new receptionists Julia Mutton and Catherine Hall. You may have met Julia at Milford whilst Catherine works at both sites.
Our very latest reception recruits are Kelly Cooper and Donna Dickie who both work at Wistaria. New receptionists have to undergo a lot of training and we hope you will be patient whilst they are finding their feet.
And finally…
Clare Richardsonis our Care Navigator, who some of you may have already met. Clare joined our team in Octoberand looks after both Wistaria and Milford patients. Her role is to provide a single point of access for patients and/or carer concerns, usually for the elderly and she can help access required services and signpost relevant information.
By assisting you to access health, social and community services your Care Navigator aims to help you maintain your independence and safety at home, avoiding hospital admissions. You can contact Clare directly, so if you have recently been in hospital or have any queries about your ongoing care or how to stay safe and independent at home please contact her on 07825 336953.
Helen Plumb, our Medicines Optimisation Pharmacist also joined us in the autumn and she too works over both sites. This is a trial post being funded by Better Local Care and the West Hampshire CCG.
Helen’s role includes medication reviews for patients on multiple medications and the frail elderly, drug monitoring and cost saving, quality and safety interventions. She will be working together with community and hospital pharmacists and GPs to improve the health and care of elderly patients. As well as helping patients it is hoped that this role will take some pressure off GPs, freeing up their valuable time.
