The successful applicant will form an important and integral part of a large multi disciplinary team providing a general adult psychiatry service to adults aged between 18 and 65 years and will work with the Community Mental Health Team and the Crisis Team. The associated inpatient beds are based at Gartnavel Royal Hospital, however, there is no in-patient component to this post. The successful applicant will be expected to work closely with CMHT and in-patient colleagues to provide a full range of services to one quarter of the Riverside catchment out-patient population suffering from psychiatric illness. In addition, the successful applicant will become a core member of the Sandyford Gender Clinic. This is the largest service of its kind in Scotland, receiving referrals from across Scotland. Gender Identity Services have recently been incorporated within a National Managed Clinical Network hosted by Greater Glasgow and Clyde which provides opportunities for networking and involvement in service developments nationally.

Applicants must have full GMC Registration, a license to practise and eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

Current Psychiatric Services – West Glasgow Area

The psychiatric services in Greater Glasgow and Clyde are split into four areas of a city wide mental health partnership (MHP); two areas North of the River Clyde and two to the South. The Mental Health partnership is responsible for the provision of mental health services for the whole of Greater Glasgow. The Associate Medical Director of Mental Health and Learning Disabilities is Dr Michael Smith who is based at the William Street Clinic. The Area Management Team for North West Glasgow Area is based at GartnavelRoyalHospital.

The North West area serves a population of around 210,000 based on 2001 census data, with an additional 60,000 at the Lomond area. The weighted population for Arndale Community Mental Health Team is approximately 54000 people aged between 18 and 65 years, based on 2001 census data. Community services in Greater Glasgow are based in Resource Centres. There are five adult resource centres in the North West area: Riverview, Arndale, Goldenhill, Shawpark and Riverside, with an additional team in Helensburgh. Community services are well established and based around community mental health teams. These services in recent years have been enhanced by the development of Crisis Teams and Primary Care Mental Health Teams.

Gender Identity Services-Sandyford Gender Clinic

Sandyford is the specialist service for sexual, reproductive and emotional health for NHS Greater Glasgow and Clyde, hosted by Glasgow City Health Partnership’s NW Sector. Sandyford provides a range of drop-in and appointment services, clinical, information and counselling services at its city centre and 18 community sites for approximately 110, 000 client visits per year.

A range of specialist reproductive health, genito-urinary medicine and counselling services are provided in addition to community gynaecology, colposcopy, vasectomy, Steve Retson Project (for men who ave sex with men) and psychosexual medicine as well as gender identity services. There are some ssertive outreach services focussing on reducing sexual health inequalities, and Sandyford also hosts Archway Glasgow, a sexual assault referral centre which provides forensic and medical services, counselling and follow-on support for those who have been recently raped or sexually assaulted.

The service covers a population of 1.1 million, has an annual budget of over £9m, and approximately 50 staff including consultants, other medical staff, nurses, sexual health advisers, counsellors, librarians, laboratory staff, managers, administrators and clerical staff. With Sandyford’s main site in Glasgow city centre, services are also delivered in 19 community hubs and satellites across the Health Board area. The gender identity service is an integrated part of Sandyford services and serves the hole of Scotland. In 2013 this service saw 275 people for a total of 403 attendances and in recent years this number has increased by approximately 10% per year. This new post is part of a wider plan to modernise Sandyford’s services to best serve this client group and meet the needs of our clients.

Overview

Arndale CMHT(5 sessions)

This post is based at Arndale Resource Centre, Drumchapel. The Resource Centre covers approximately a population of 54000, covering a range of housing types and deprivation levels. The team comprises community psychiatric nursing, occupational therapy, psychology and social work and managed by the Service Manager.

It is expected that the successful applicant would work closely with the Community Mental Health Team, Crisis Team, Service Manager, Social Work and voluntary organisations to provide a comprehensive service working as a part of a multi disciplinary team. In-patient beds are based at GartnavelRoyalHospital although this job is based predominantly in the CMHT.

Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in General Adult Psychiatry or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

It is desirable that a successful applicant would be an approved medical practitioner in terms of the Mental Health (Scotland) Act 2003. It is desirable that the successful applicant has membership of the Royal College of Psychiatrists and has a minimum of 3 years Post Graduate experience working in Psychiatry.

Domiciliary visits, Mental Health Act / Adults Incapacity and legal work arising from the case load assigned to the successful applicant would be expected to undertaken.

The postholder will also cover colleagues in line with the arrangements set out in the National Terms and Conditions.

The postholder will be supported by input from the In-patient consultants and will be expected to closely liaise with the in-patient staff for patients with whom the care is shared. There is additional cover in the resource centre from junior doctors.

Sandyford Gender Clinic (5 Sessions)

The Service sees patients from across Scotland and is the largest centre for the treatment of Gender Identity Disorders in the country. The role of the Clinic is to provide assessment and co-ordination of treatment for patients with Gender Identity Disorders, most commonly transsexualism. The clinic adheres to the National Standards as set out in the Gender Protocol for Scotland, 2013 and the World Professional Association for Transgender Health’s, Standards of Care, 2011. The Service has also recently been funded to host the National Clinical Network for Gender Services in Scotland.

This post is based at Sandyford which also hosts Glasgow’s Sexual health Services. The post holder will primarily be responsible for the assessment and treatment of Gender Identity Disorders in patients aged over 17 years of age. This entails thorough psychiatric assessment, exclusion of significant co-morbidity and management or onward referral of significant mental health difficulties. There is a significant amount of liaison with other specialties and agencies including endocrinology, surgical disciplines and social services amongst others.

The consultant works as a core member of the multidisciplinary team so as to provide a comprehensive assessment and treatment service to these patients. Active participation in regular team meetings and combined clinics is expected.

It is desirable that the candidate would have some experience of working with this client group and be able to demonstrate an enthusiasm for the work. Further training will be provided based on individual learning needs. CPD activities in this specialist field will be supported.

On Call Commitments

The on call commitment is currently 1:40 with responsibility for the cover of the North Glasgow area supported by a 1st and 2nd on-call doctor. An availability supplement of 3% is payable for this work.

3. Duties of the Post

Arndale CMHT

The main clinical responsibilities are the ongoing management of out-patients with severe and enduring mental illness and assessment and management of new referrals including emergency/crisis referrals. This is organised through weekly Out Patient Clinics together with acute work and domiciliary visits as required to provide an emergency response and urgent advice to patients and their GPs. There is an ongoing process of continuing patient review at Arndale to which the Consultant would be expected to contribute. In this catchment area there is a diverse population giving the opportunity to become involved with the assessment and treatment of a wide variety of mental disorders.The post holder will be expected to provide additional medical leadership to the team and assist with the further development of the service. There is excellent support from a skilled and motivated multi-disciplinary team.

Flexibility of the post holder is necessary to provide medical input for the CMHT. This may involve urgent assessments of patients, and urgent advice to colleagues regarding patient assessment and management. Cover for the leave of colleagues will be necessary.

Sandyford Gender Clinic

As part of the multi disciplinary team at Sandyford, the postholder will manage patients with gender identity disorders in line with current treatment protocols. The service only provides out-patient treatment involving the assessment of gender identity disorders and ongoing support and management of patients aged 17 and over. Referral to other agencies and services across Scotland is required to facilitate treatment. Liaison with other specialties and services is important and engaging with families, patient advocacy groups and outside support agencies is often required.

There are no on-call arrangements for this part of the job. Cross cover of colleagues is expected.
