Iao School Renaissance Scholarship

Instructions

Students may apply for more than one subject area but may be a first place recipient for only one subject area scholarship. However, a qualifying eighth grade male and female student will also be a recipient of the Outstanding Renaissance Student Award.

Checklist:

_____ 1. Print legibly (use black ink) or type when filling out the application form.

_____ 2. Fill each section as thoroughly as possible.

_____ 3. Provide your social security number in the space provided.

_____ 4. Write your name and teacher’s name on Teacher Recommendation Form (minimum two) before asking teachers to complete

the recommendation form. One of the teacher recommendations must be from a present or past teacher in that subject area. The teacher may either give their completed recommendation form directly to Mrs. Higa or to the student. If any recommendations are missing, Mrs. Higa will notify the student.

* Distribute recommendation forms for teachers to complete by: February 2nd

**Teachers will have the right to refuse any forms given to them

after February 2nd.

_____ 5. Include one Non-Teacher Recommendation Form.

_____ 6. Attach copies of your final/current report cards. (Grades 6, 7, & 8)

Grade 6 – current report card

Grade 7 – final grade six report card and current report card

Grade 8 – final grade six and seven report card and current report card

_____ 7. Be sure that your parents sign the application.

_____ 8. Turn in completed application(s) to Mrs. Higa at the front office.

_____ 9. Observe the DUE DATE. The due date is Friday, February 18, 2011.

Note: If you are applying for more than one subject area, please submit a separate packet for EACH subject area. This packet is only for ONE subject area. You will need to make copies of the given packet if you are applying for more than one subject area.
