FORM C – Project Narrative
Using this outline as a template, provide a narrative description of your proposed project. When completed, save as a Word document and upload as part of your grant application.
- Need: Describe and document the problem that you propose to address and how many individuals in the target area are affected by it. Provide relevant local data or citations supporting the need and whether the services or activities that you propose are currently provided in the MetroWest area.
- Program Design: Describe in detail the elements of the project for which funding is being requested. Who and how many will be served? What specific strategies will you use to recruit participants? What barriers to success do you anticipate for the program? If this is a request for a continuation grant, please document the progress of your current grant, including the number served and the outcomes achieved so far.
- Organizational Capacity: Describe the applicant organization and its core services. Why are your organization and staff best qualified to undertake this project? Who are the people that will lead this effort and what factors most predict their ability to achieve success?
- Outcomes: What are the specific outcomes you expect to achieve? What is the anticipated timeline for achieving these results? Please be sure your outcomes are consistent with those listed in the Abstract/Logic Model section of the application.
- Tracking Success: How will you know when your results have been achieved? What information will you generate and use to track success and make corrections in your program? What do you most want to learn from this project? Please be sure the indicators and measurements correspond to those identified in the Logic Model.
- Disparities: How will your organization address the racial/ethnic disparities and economic inequities experienced by those to be served through this proposal? What challenges has your organization encountered in addressing these issues?
- Collaboration: Are there any partners or intermediaries critical to your success? If so, please list them and describe what role they will play. Include letters of collaboration. Note: A letter of collaboration signed by the School Superintendent must be submitted for all proposals involving schools.
- Sustainability: Describe how you intend to sustain the work of the project once Foundation funding ends, keeping in mind that sustainability means more than replacing funding. What is your organization’s track record in sustaining other projects?
