Team / Please use the team name as designated in the NVYLL schedule, i.e. U15UB_2, G78UB, U9MF_3
First Named / First name of the player or coach
Last Name / Last name of the player or coach
Coach / Head Coach (HC) or Assistant Coach (AC)
Position / We are trying to identify players and coaches here. Field position is not what we are after. So use Head Coach, Asst. Coach, Player
Gender / Boy or Girl
Age / Current Age for players only. Coaches ages are not necessary
Address / Address line for person
City / City line for person
State / State line for person
Zip Code / Zip code for person
Phone / Phone number for person
Birth date (month and year) / Birth date for players only. Coaches ages are not necessary
Email address / Email address for person
Grade / Grade in school for players
High School / Pyramid high school that the player would attend.
