The 2016 Extraordinary Woman Award sponsored by State Representative Warren Kampf recognizes one exceptional woman who resides within the 157th Legislative District (Tredyffrin Township, Upper Providence Township (parts), Phoenixville Borough (parts), Schuylkill Township) who has demonstrated exceptional achievement, personal excellence or outstanding service.
The award winner will be announced at State Representative Warren Kampf’s 2016 Women’s Luncheon on Friday, October 14, 2016 at the Pfizer Conference Center, 500 Arcola Road, Collegeville.
To nominate someone for the 2016 Extraordinary Woman Award, please fill out and submit the nomination application. All nominations are due Friday, September 2, 2016 by 5 p.m.

Please complete the following application in its entirety. Incomplete applications will not be reviewed. This application is formatted to allow for editing in the text regions or it can be printed and completed by hand. Mail or drop off the completed application to:

If mailing, applications must be postmarked no later than Friday, September 2, 2016. The nominee MUST reside within the 157th legislative district. In the essay portion on page 2 (section 3), please clearly articulate your reasons for nominating this individual. Kindly keep the essay response to no more than 750 words. The award winner will be chosen by Rep. Kampf and his staff. The award winner will be notified in advance of the award luncheon to verify attendance. Luncheon information will be available on Rep. Kampf’s website www.repkampf.com. All women of the 157th Legislative District are invited to attend the luncheon and RSVP is required.
