All students wishing to attend the dance and bring a non-Riverside High School guest (only one guest per student) must fill out and submit this form by Wednesday, October 25, 2017. In order to give the RVHS Administration ample time to look over and approve this guest form, the due date is not flexible, and the school will not approve late guest forms.

GUEST BEHAVIORAL COMMITMENT
All students attending a Riverside High School activity, on or off campus, are expected to know appropriate behavior is required and that all school rules are enforced at any activity sponsored by Riverside High School.
- All school rules and policies (including the dress code) apply to school activities. See the Student Handbook for info.
- All guests must be 20 or under and not a middle school student. No exceptions.
- All guests mustattach a legible copy of the guest’s picture ID from their current school or their driver’s license to this form.Guests must have a picture ID to get in the door.
- All attendees must arrive by 9:00 PM and all Guests MUST arrive with the RVHS STUDENT. Guest will not be admitted without the RVHS Student accompanying him/her.
- Dress must be appropriate for semi-formal/formal events.
- The Guest, the RVHS Student, and their parents will be held responsible for the behavior of the guest. Students and guests will be asked to leave if they do not comply.
- The school reserves the right to refuse entrance to the dance of any RVHS student or guest.
- If a guest or student is sent home for not abiding by this contract, no refund will be given.
- Once you leave the dance, you may not re-enter.

Furthermore, I have read and understand the above rules pertaining to behavior at school events. I understand the consequences for any inappropriate behavior.
