There are three steps to follow to calculate and authorize your monthly donation:

The first will be to decide the amount of your monthly pledge for normal parish activities.

Please show this amount at the bottom of this page.

The second step will be to show the amount you wish to specify for special collectionsfor each month. These amounts should be shown on the next page.

Thethird step is to return this form, together with a blank cheque marked VOID. You may put this in the Sunday collection basket in a sealed envelope addressed to Jean Goddeeris, (Holy Angels’)/ Maureen Marcelissen (St. Anne’s) or you may bring it or mail it to the office-20 Morrison Drive, St. Thomas, Ontario N5R 4S6.

**A phone call to the parish office can change your donation at any time. To have a contribution stopped completely please call 20businessdays prior to withdrawal date.

All information will be handled with strictest confidence.
