Effect of Soybean Protein on Blood Pressure: A Randomized, Controlled Trial

The authors examined the effect of soybean supplements to the diet in 302 participants with an initial untreated systolic blood pressure of 130 to 159 mm Hg, diastolic blood pressure of 80 to 99 mm Hg, or both. Systolic and diastolic blood pressure both decreased. Increasing soybean protein intake may help to prevent and treat hypertension.

An Editorial Update: What Level of Blood Pressure Control in Chronic Kidney Disease? Andrew S. Levey and Cynthia D. Mulrow
Several recent studies have tried to characterize the effects of a lower-than-usual target blood pressure on progression of nondiabetic kidney disease. An observational study, the Modification of Diet in Renal Disease Study, showed long-term benefit. The randomized African-American Study of Kidney Disease and Hypertension showed no benefit, nor did the recently published Ramipril Efficacy in Nephropathy 2 Trial. Here's our take on these conflicting study results.

LA NCEPOD ha recentemente pubblicato uno studio che ha riguardato la gestione dei pazienti ricoverati in Inghilterra e nel Galles con diagnosi di Insufficienza renale acuta; lo studio ha mostrato che il 50% dei pazienti non riceve uno standard accettabile di cure; nel 43% dei casi vi è un "inaccettabile" ritardo nel riconoscere la condizione. Secondo gli advisors almeno 1/5 delle IRA diagnosticate dopo il ricovero erano prevedibili e quantomeno evitabili. Le complicanze della IRA erano state omesse nel 13% dei casi, evitabili nel 17% o gestite scorrettamente nel 22%.
Tra le raccomandazioni espresse nelle conclusioni vi è la necessità di eseguire un check per la presenza di IRA in tutti i pazienti ammessi in un dipartimento di emergenza, valutando gli elettroliti all'ammissione e successivamente con adeguata tempistica; le IRA prevedibili o evitabili non devono aver luogo in ogni caso. Per quei pazienti ricoverati che dovessero sviluppare IRA si dovrebbero valutare con precisione tutti i fattori di rischio che possano contribuire al suo sviluppo ed evitare con decisione tutte le complicanze collegabili ad essa. Una raccomandazione anche per l'Università e le scuole di specializzazione che dovrebbero sempre includere nei loro programmi la prevenzione, diagnosi e trattamento dell'IRA.
