The Program will be coordinated by Healthyroads, an Internet-based, personal health improvement program, and we will be launching it on [LAUNCH DATE]. Here is the program overview:

THE GOALS OF THE PROGRAM ARE TO:

·  Provide you with information about your current health status.

·  Help you set realistic wellness goals.

·  Arm you with tools and resources to help you reach your goals.

WHO CAN PARTICIPATE?

·  All employees and spouses of [FIRM NAME] are eligible to participate and will have access to all of the features of the web portal and biometric screenings.

·  Children and/or adult dependents cannot participate in the program.

·  Your participation in the program is voluntary.

INCENTIVE:

·  Employees and spouses who are covered under our health insurance plan AND complete the required program elements by [END DATE] will qualify for a $150 incentive.

·  To earn an incentive covered employees and spouses must complete the Personal Health Assessment, Biometric Screening, and earn 30 points (see chart below).

·  Employees and spouses not covered under our health plan may still utilize all the features of the program however, no incentive is available.

REGISTERING FOR THE PROGRAM:

·  Visit www.healthyroads.com and click on “Create Account”

·  Input your first name, last name, and date of birth

·  If you have any problems with creating your account, please call the Healthyroads customer service line at 1-877-330-2746

PERSONAL HEALTH ASSESSMENT:

·  Used to help you identify your current health status and any potential issues and risks

·  60-70 questions , Takes 20-30 minutes to complete

·  Questions change depending on how you respond

·  Stored online in personal account for your review

BIOMETRIC SCREENINGS:

Biometric screenings can be completed in one of two ways:

Visit a Quest Diagnostic Patient Service Center (PSC): / OR / Visit your Primary Care Physician
·  Schedule and appointment on My.BluePrintForWellness.com using the registration key ACEC2015.
·  Your Unique ID is: The first three initials of your first name + last name + first four digits of your date of birth.
Example:
Charlie Brown 04/01/1960 = chabrown0401.
·  Quest screenings are offered at no charge and include: measurements of your cholesterol and blood glucose (sugar) levels via a venipuncture blood draw, blood pressure reading, Body Mass Index (BMI) calculation from your height and weight.
·  Participants visiting Quest centers are encouraged to bring a printed copy of their scheduling confirmation page.
·  Lab results will be mailed within 21 days and will be loaded in each participant’s personal profile at www.Helathyroads.com within 30 days. / If you haven’t had your annual preventive exam this year yet: You have the option of completing a screening with your Primary Care Physician and then having your physician complete the Biometric Screening Results-Physician Form on your behalf.
·  The form must be faxed, e-mailed or sent via regular mail to Healthyroads. Instructions are available on the form, and the form can be downloaded from www.healthyroads.com
If you already had your annual preventive exam this year:
·  If your visit occurred on [Launch Date] or after, you may use your test results to complete the Biometric Screening Results-Physician Form. Contact the HR Department for details.
·  If your exam occurred prior to [Launch Date], you may want to complete your screening at a Quest Diagnostic Patient Center to avoid incurring a charge under the health insurance plan.

CONFIDENTIALITY:

·  The program is on a secure web portal and is HIPAA compliant to protect your personal health information.

·  The ACEC Life/Health Trust, United HealthCare and [FIRM NAME] will NOT receive any individually identifiable medical information.
