Getting the Letters Out on Time

Please submit this questionnaire with your graduate school application materials. This includes:

1. any forms the schools require that the recommender complete

2. student copy of your transcripts

3. stamped, addressed envelopes

4. a summary sheet listing each school you are applying for and the address to which the recommendation must be sent.
