1. This SVdP Conference must be officially aggregated Aggregation Date
2. This SVdP Conference has an IRS 501 (c) (3) letter and is in good standing EIN#
3. This application must be signed by a District Council or Diocesan Council President (not both)
4. If funded, you must agree to submit a detailed financial accounting of all grant funds within one year. The National Council reporting form must be used.
5. This request does not exceed the current grant limit of $5,000
These items MUST be included in your application or your application is considered incomplete
6. Enclosed is a Project Narrative (a detailed explanation of the project and its impact)

B. Purpose of Grant – overview of how funds will be used and its impact
C. Line Item Budget Narrative – written narrative in dollar format of specifically how the funds will be used
7. Enclosed is the Conference’s latest annual report (must be last fiscal year; no account statements or balance sheets will be accepted)

9. Funds Raised from Friends of the Poor® Walk
