I hereby make application to the Professional Phase of the following program for the semester.
I understand:
- After my application has been reviewed and I am accepted to Professional Phase, my status will be changed to HLTH or KINE. (This can be checked on howdy.tamu.edu)
- I will be allowed to register for Professional Phase classes during my assigned registration time.
- If I fail to meet ALL admission requirements to the Professional Phase for my option (including transfer coursework from other institutions) by one week after Texas A&M University’s grades are posted, then I will be dropped from professional phase classes and my status will be changed back to lower level (EDHL or EDKI). NO EXCEPTIONS!!
- I must make a “C” or better in all my science foundation, professional development, and major courses.

Note: Conviction of a felony or misdemeanor for a crime which directly relates to the duties and responsibilities of the teaching profession may prevent certification.

**Final Portfolio Review must be completed by the Q-drop deadline of semester enrolled in KNFB 416 or HLTH 415.
I understand I must:
- Meet the requirements of a minimum overall GPR of 2.50 and degree plan GPR of 2.50 to register for my pre-student teaching class(es) the full semester before you student teach. These classes are not offered in the summer.
- In order to student teach, I must make a “C” or better in all science foundation, professional development, major and support field elective courses. I must also have a minimum GPR of 2.5 in all of the following areas:
a) Overall TAMU, b) degree plan, c) Major, and d) professional development.
- Transfer all credit hours taken at another institution to TexasA&MUniversity’s Office of Admissions and Records.
The semester you are enrolled in KNFB/HEFB 450 (student teaching), you will be required to apply for graduation at howdy.tamu.eduwithin the first two weeks of the semester and pay the diploma fee.
