This form must be completed and turned inat the time of ticket purchase. Tickets will not be sold after April 30, 2015 - NO EXCEPTIONS!

PLEASE REMEMBER TO BRING YOUR ID WITH YOU TO THE DANCE.

*Guests must be at least in the 9th grade and NO GUESTS 21 YEARS OF AGE AND OLDER WILL BE ADMITTED. Guests will not be permitted to enter the event without prior approval. Only one guest per student is allowed. All guests must present a photo ID at the door.

If the guest is out of high school, attending college or working, the parent/guardian of the guest must assume responsibility for the guest behavior.

Guests to Clear Creek High School events are expected to show anID upon entrance; behave and dress appropriately for attendance and participation. Costumes, masks or other inappropriate attire are NOT allowed. It is the expectation that all guests of functions and activities of Clear Creek High School demonstrate both behavior and dress which is appropriate to the activity or event. Students and/or their guests at a school function will not be allowed to display negative behavior or behave in a manner or dress that would disrupt the event and/or dishonor other students. Clear Creek High School reserves the right to deny admission to any guests who do not follow these guidelines. In addition, Clear Creek High School reserves the right to ask guests to leave if their behavior is inappropriate.

Clear Creek High School reserves the right to search any tote, bag, duffel bag or purse that is in possession of a student or guest if issue warrants.
