To All Nominating Staff and Faculty Members:
Eligibility: In order to be eligible, a student must graduate between June 2016 and May 2017 and have a minimum 3.5 GPA.
Step One: Complete and submit the below nomination form to Anastasia Falcheck, Committee Chair, Bush Hall 124 or email at . It is strongly recommended that you work closely with the student you are nominating to ensure that your nomination is as complete as possible, and to coordinate Step Two of the process.
Step Two: Submit to the Committee Chair supporting materials--the student's resume, a co-curricular transcript, and two references from faculty or staff. If a student needs to develop or update a co-curricular transcript, refer them to the Student Activities Office in Farrell Center.
The application form and supporting materials are due Friday, December 2, 2016.
Step Three: SUNY Delhi can nominate two students for these awards based on campus enrollment. The SUNY Delhi campus Chancellor’s Award for Student Excellence selection committee will review all campus nominations and will recommend two students to the College President to move on in the selection process.

The prestigious Chancellor’s Award for Student Excellence acknowledges students who have best demonstrated and been recognized for their integration of academic excellence with other aspects of their lives, which may include: arts, athletics, career achievement, community service, campus involvement, and leadership.
Step by Step Process:
Part I: Academic Excellence
Please provide rationale for the nomination of the student and note any awards or recognitions with dates and descriptions.
Part II: Extra-curricular Accomplishments
Students nominated must have integrated academic excellence with
extra-curricular accomplishments which may include:
- arts
- athletics
- career achievement
- community service
- leadership
-campus involvement
Please describe in detail the student’s contributions for each relevant area listed above and note any awards or recognitions with dates and descriptions.
