PROGRESS REPORT INSTRUCTIONS:

USING THE SCIENCE CORE TEMPLATE TO PREPARE YOUR APPLICATION:

·  Save an electronic copy of this template titled with your Core’s name

·  Download the guidance document for all cores for definitions and further explanation of information requested for this template

·  Other Helpful Information is available for download on the progress report webpage (i.e., CFAR membership listing with NIH categories and CFAR Support Table for Developmental and other awardees, and CFAR Funded Research Base Table, 2013)

·  Enter your data and information into this template in the areas indicated (black font areas)

·  Reporting dates for the report and user tables are: 06/01/2014-05/31/2015 or the 1st of a 5yr period. The only exceptions are the Developmental Core and the Developmental Core appendices. Also if there are any really big things that happened in the year before the renewal funded, add those to the report.

·  Remove all instructions to the preparer once completed (delete text in red font)

·  Complete the user table (downloadable from website) and submit with this template; separate guidance for all user tables is available for download on the webpage.

·  Bold all projects listed on the Funded Research Base that appear in your Core’s User Table

·  Submit the narrative template and the user table as separate word files by the deadline to Donna Porter, Avni Doshi and Michael Saag for preparation of the Director’s Overview and uploading to eRA Commons
