Week 1 ___ Orientation to HEALTHSOUTH

___ Orient to Facility

___ Review with supervisor Facility scheduling

___ Review notebook and assignments/ from facility and from

school assignment

___ Review Patient charts

___ Establish a clinical supervisor meeting time daily

___Begin student notebook, diagnostic list

___Review activity analysis

___Review Occupational Therapy treatment:

Approaches /Biomedical/Neurodevelopment/Rehabilitative

___Review basic writing skills SOAP form/ write daily progress

SOAP form on separate paper for supervisor to review. Follow

SOAP note form and add facility requirements.

___Complete any assignments for school and submit a copy to

Supervisor

___Meet with Supervisor at end of week and establish next week

goals based on assignments

___Review FIMS scoring

Week 2

___ Develop treatment ideas for 1 patient/ ortho/ and set goals

___ Assignment of 1 patient/ ortho/ observation and assistance in

Treatment

___ Identify strengths and problem list/ Establish goals for selected

patient

____Write daily SOAP notes for assigned patient and others as

Assigned

____Observe Other Disciplines ST/ PT

____Attend rehab meetings and conference

____Complete any Assignment for school and submit a copy to

Supervisor

____Observe evaluation process and documentation required

____Maintain Notebook/

____Meet with Supervisor review assignments and establish next week

Goals

Week 3

____ Complete an activity analysis/Submit to supervisor

____ Perform a client centered Interview/ Review with

supervisor/submit

____Choose your case study patient and Presentation date

____ Perform a chart review

____Select 3 patients to evaluate, develop a treatment plan, provide

treatment, write daily notes, weekly progress note, and discharge

summary

____Report on these patients in conference

____Meet with supervisor and discuss Final week goals and

assignments/ strengths and weaknesses

Week 4

___Follow one patient throughout his/her day, recordings patient

Activities

___Attend all student in-services

___Case presentation

___ Maintain a 4 patient case load of closed supervised activities

___Complete school assignments/ submit copy to supervisor

___Spend ½ day in other gyms/observation
