Precleaning
- Recognizes that flexible endoscopes and accessories should be precleaned at the point of use.
- Precleans flexible endoscopes and accessories at the point of use as soon as possible after the endoscope has been removed from the patient (or the procedure is completed) and before organic material has dried on the surface or in the channels of the endoscope.
- Performs precleaning in accordance with the endoscope manufacturer’s instructions for use (IFU) and by
- preparing a fresh solution of the cleaning product with properties recommended by the manufacturer;
- washing the exterior surfaces of the endoscope with a soft, lint-free cloth or sponge saturated with the cleaning solution;
- suctioning the cleaning solution through the suction and biopsy channels;
- placing the distal end of the endoscope in the cleaning solution and suctioning the solution through the endoscope;
- flushing the air, water, and other channels of the endoscope alternately with the cleaning solution and air, finishing with air;
- visually inspecting the endoscope for damage; and
- discarding the cleaning solution and cleaning cloth or sponge after use.
- Wipes the external surfaces of the endoscope with a soft, lint-free cloth or sponge saturated with utility or sterile water and suctions water through the channels when the precleaning process will be delayed.
Transporting
- Transports the flexible endoscope and accessories to the endoscopy processing room as soon as possible after use.
- Keeps the endoscope wet or damp, but not submerged in liquid, during transport.
- Transports contaminated endoscopes and accessories to the decontamination area in a closed container or closed transport cart.
- Uses a container that is leak proof, punctureresistant, and large enough to contain all contents.
- Uses a container that is of sufficient size to accommodate the endoscope when the endoscope is coiled in large loops.
- Labels the transport cart or container with a biohazard legend.
- Securely affixes the biohazard label to the cart or container.
- Transports the accessories with the endoscope but contains them separately from the endoscope.
- Begins to process flexible endoscopes and accessories (or recognizes that processing should begin) as soon as possible after transport to the endoscopy processing room or within the manufacturer’s recommended time to processing.
- When it is not possible to initiate the cleaning process within the endoscope manufacturer’s recommended time to cleaning, follows the manufacturer’s IFU for delayed processing (or recognizes that these IFU should be followed).
- Does not leave flexible endoscopes soaking in enzymatic cleaning solutions (or recognizes that endoscopes should not be left in enzymatic cleaning solutions)beyond the endoscope manufacturer’s designated contact time unless this is recommended in the manufacturer’s IFU for delayed processing.
- Records the time that the procedure was completed and cleaning was initiated per [facility-specific protocol].
- Cleans and disinfects transport carts or containers per [facility-specific protocol].
Leak Testing
- Recognizes that flexible endoscopes designed to be leak tested should be leak tested after each use, after any event that may have damaged the endoscope, and before use of a newly purchased, repaired, or loaned endoscope.
- Performs leak testing before manual cleaning and before the endoscope is placed into cleaning solutions.
- Performs leak testing in accordance with the endoscope and leak-testing equipment manufacturers’ IFU and by
- removing all port covers and function valves;
- placing the endoscope in a loose configuration;
- pressurizing the endoscope to the recommended pressure;
- manipulating all moving parts, including the elevator, and angulating the bending section of the distal end;
- actuating video switches; and
- maintaining pressure and inspection for a minimum of 30 seconds.
- When an endoscope fails a leak test, removes it from service and sends for repair or replacement per [facility-specific protocol].
Manual Cleaning
- Recognizes that after leak testing and before high-level disinfection (HLD) or sterilization, flexible endoscopes should be manually cleaned.
- Begins manual cleaning as soon as possible after leak testing.
- Performs manual cleaning in accordance with the endoscope manufacturer’s IFU.
- Performs manual cleaning using the type of water and cleaning solution recommended by the endoscope manufacturer.
- Contacts the endoscope manufacturer for recommendations for other cleaning solutions that may be used if the manufacturer’s recommended cleaning solution is not available.
- Performs manual cleaning using a freshly prepared cleaning solution.
- Changes the cleaning solution before it become cloudy or discolored and before there are visible particles in the solution.
- Does not add additional products to the cleaning solution unless this is recommended by the manufacturer.
- Follows the cleaning solution manufacturer’s IFU for
- water quality, hardness, and pH;
- concentration and dilution;
- water temperature;
- contact time;
- conditions of storage; and
- use life and shelf life.
- Changes the cleaning solution when the temperature of the solution does not meet the temperature specified in the manufacturer’s IFU.
- Completely submerges the endoscope in the cleaning solution during the cleaning process.
- Detaches removable parts (eg, valves, buttons, caps) from the endoscope and submerges them if recommended by the endoscope manufacturer’s IFU.
- Cleans all exterior surfaces of the endoscope with a soft, lint-free cloth or sponge saturated with the cleaning solution.
- Cleans all accessible channels and the distal end of the endoscope with a cleaning brush of the length, width, and material recommended by the endoscope manufacturer.
- Manually actuates the endoscope valves while cleaning.
- Cleans and brushes the elevator mechanism and the recesses surrounding it with a cleaning brush of the length, width, and material recommended by the endoscope manufacturer.
- Raises and lowers the elevator mechanism throughout the manual cleaning process.
- Uses a clean brush for each endoscope cleaning.
- Visually inspects brushes and other items used to clean endoscope channels before use and does not use them if the integrity of the brush or other cleaning item is in question.
- Brushes the accessible channels of the endoscope multiple times until no debris appears on the brush.
- Removes debris from the brush before the brush is retracted back through the channel and after each pass by swirling the brush in the cleaning solution and rinsing it.
- Flushes the channels of the endoscope with cleaning solution.
- Flushes and rinses the exterior surfaces of the endoscope with utility water until all cleaning solution and residual debris is removed.
- Dries the exterior surfaces of the endoscope with a soft, lint-free cloth or sponge and purges all channels with instrument air.
- Cleans, brushes, rinses, dries, and high-level disinfects or sterilizes all reusable parts (eg, valves, buttons, port covers, tubing, water bottles), accessories (eg, forceps) and cleaning implements (eg, brushes, channel cleaning adapters).
- High-level disinfects or sterilizes water and irrigation bottles at least daily.
- Does not allow any residual water or moisture to remain in the water bottle assembly.
- Uses sterile water to fill water and irrigation bottles.
- Cleans and processes insulated electrosurgical devices used during endoscopic procedures in accordance with [facility-specific policy].
- Discards single-use parts, accessories, and cleaning implements after each use and does not reprocess.
Inspecting
- Recognizes that flexible endoscopes, accessories, and associated equipment should be visually inspected for cleanliness, integrity, and function before use, during the procedure, after the procedure, after cleaning, and before disinfection or sterilization.
- Visually inspects and processes according to the manufacturer’s IFU all new, repaired, refurbished, and loaned endoscopes, accessories, or other equipment before use.
- Visually inspects and evaluates endoscopes, accessories, and equipment for
- cleanliness,
- missing parts,
- clarity of lenses,
- integrity of seals and gaskets,
- moisture,
- physical or chemical damage, and
- function.
- Uses lighted magnification to inspect endoscopes and accessories for cleanliness and damage.
- Uses an endoscopic camera or borescope to inspect the internal channels of flexible endoscopes.
- Removes defective endoscopes, accessories, and equipment from service and sends for repair or replacement per [facility-specific protocol].
High-Level Disinfection or Sterilization
- Recognizes that following manual cleaning and when compatible with the endoscope manufacturer’s IFU, flexible endoscopes and accessories should be either mechanically cleaned and mechanically processed by exposure to a high-level disinfectant or liquid chemical sterilant or mechanically cleaned and sterilized.
- Performs mechanical processing in accordance with the endoscope manufacturer’s IFU and the mechanical processor manufacturer’s IFU.
- Verifies compatibility between the endoscope and the mechanical processor before processing.
- Positions the endoscope and accessories within the mechanical processor in a manner that ensures contact of the processing solutions with all surfaces of the endoscope.
- Ensures all connectors between the endoscope and the mechanical processor are connected correctly.
- Monitors the mechanical processing cycle to verify it is completed as programmed.
- Repeats or recognizes that if a mechanical cycle is interrupted the entire cycle should be repeated.
- Performs mechanical processing using critical water.
- Performs mechanical processing using the cleaning, disinfectant, and sterilant solutions and chemicals recommended by the endoscope manufacturer and the mechanical processor manufacturer.
- Handles chemicals and solutions used for cleaning and processing flexible endoscopes and endoscope accessories in accordance with local, state, and federal regulations and the manufacturer’s IFU.
- Verbalizes the location of the safety data sheets for chemicals used for cleaning and processing flexible endoscopes.
- Verbalizes the location of the chemical spill kit.
- Does not use the following products for processing flexible endoscopes:
- skin antiseptics,
- hypochlorites,
- phenolics, and
- quaternary ammonium compounds.
- Following disinfection, mechanically rinses and flushes the endoscope and endoscope channels with critical or sterile water.
- Rinses endoscope accessories and removable parts with critical or sterile water.
- Flushes endoscope lumens with 70% to 90% ethyl or isopropyl alcohol per [facility-specific protocol].
- After mechanical processing, dries the exterior surfaces of the endoscope with a soft, lint-free cloth or sponge.
- Purges the endoscope channels with instrument air or the mechanical processor drying system.
- Dries all removable parts and endoscope accessories.
- Packages and sterilizes endoscopes and endoscope accessories in accordance with [facility-specific policy].
Storing
- Recognizes that flexible endoscopes and accessories should be stored in a manner that minimizes contamination and protects the device or item from damage.
- Stores flexible endoscopes in accordance with the endoscope and storage cabinet manufacturers’ IFU.
- Does not store flexible endoscopes in their original shipment cases.
- Stores flexible endoscopes with all valves open and removable parts detached, but stored with the endoscope.
- Clearly identifies processed endoscopes by [facility-specific visual cue].
- Visually inspects endoscopes and storage cabinets for cleanliness before placing endoscopes into or removing them from the cabinet.
- Reprocesses the endoscope before use if there is any evidence of contamination of the endoscope (eg, soil, moisture).
- Removes and reprocesses all endoscopes in the cabinet and cleans the cabinet if there is any evidence of contamination of the cabinet (eg, wet spots, soil, fecal odor).
- Wears clean, low-protein, powder-free, natural rubber latex gloves or latex-free gloves when handling processed endoscopes and when transporting them to and from the storage cabinet.
- Recognizes that sterile gloves are not required for handling processed flexible endoscopes unless the endoscope is intended to be placed on a sterile field.
- Removes and reprocesses the endoscope if [facility-specific maximum storage time] has been exceeded.
- Cleans and disinfects the storage cabinet used for flexible endoscopes according to [facility-specific policy].
- Stores sterile items in a sterile storage area per [facility-specific policy].
Recording
- Records (or recognizes that records related to processing flexible endoscopes should include) the
- date and time,
- identity of endoscope and endoscope accessories,
- method and verification of cleaning and results of cleaning verification testing,
- number or identifier of the mechanical processor or sterilizer and results of process efficacy testing,
- identity of the persons performing the processing,
- lot numbers of the processing solutions,
- disposition of defective items or equipment, and
- maintenance of water systems, endoscopes and endoscope accessories, and processing equipment.
- Records (or recognizes that records related to flexible endoscopy procedures should include) the
- date and time,
- identity of the patient,
- procedure,
- identity of the licensed independent practitioner performing the procedure, and
- identity of the endoscope and endoscope accessories used during the procedure.
- Verbalizes a review of facility or health care organization policies and procedures related to processing flexible endoscopes.
- Participates in assigned quality improvement activities related to processing flexible endoscopes.
