Pho Sure’s4rdAnnual Eating Contest

What:Pho eaten in 10 minutes.

Where:Pho Sure Valencia. 23876 Copper Hill Drive, Valencia.

When: September 19th, 2015 at 3:00PM

To Enter:$10 entry fee and a completed form submitted by deadline. Only the first 15 contestants will compete.

RULES:

Contestants must be at least 18 years old at time of competition.

Contestants cannot begin to eat until starting signal is given.

Whole bowl with broth must be completely finished in order to count.

Contestants must stop at the end of 10 minutes.

Visible symptoms of illness will disqualify contestant.

Pho Sure has the sole, complete and final discretion regarding all contest related matters including basis for disqualifications and victory.

The contestant consuming the most PHOwith meats and broth or portion thereof shall be deemed the winner. Winner shall receive a $100 gift card, a trophy and the admiration and adoration of the Vietnamese and American people everywhere.

WAIVER & RELEASE OF LIABILITY

I, the undersigned, know that the before listed event is a potentially hazardous activity with risks of personal injury, illness and potential death and I attend it out of my own free will and choice. In choosing to participate in the Pho Sure’s Annual Eating Contest and any related activities, I fully accept and assume all risks whether before, during or after this contest and its related events. I waive any and all specific notice of the existence of the risks. I shall assume and pay my own medical and emergency expenses in the event of injury, illness, or other incapacity regardless of whether I authorized such expenses.

I realize that this activity requires physical conditioning and I represent that I am in sound medical condition capable of participating in the contest without risks to myself or others. I have no medical impediment, which would endanger others or me. Knowing these facts and in consideration of my entry acceptance, admission to and/or participation in the Pho Sure’s Annual Eating Contest and its related events, I for myself and anyone acting on my behalf, release, waive, discharge, covenant not to sue and agree to hold Pho Sure, its owner(s), agents, representatives, employees, affiliates, sponsors, emergency and support personnel, volunteers and their representative harmless from any and all claims, demands and actions of any and every kind. I have, may have or may hereafter accrue against the released parties directly or indirectly arising out of or relating in any respect to my attending or participating in the Pho Sure’s Annual Eating Contest and its related events.

In the event of an emergency, I do hereby authorize and consent to any medical attention or services necessary.

Additionally, I agree to allow Pho Sure to publish my name and use any photographs taken of me for the purpose of promoting the event in the future without compensation.

Final Authority: Pho Sure has the sole, complete and final discretion regarding all contest related matters including basis for disqualifications and victory.
