If a Disclosure and Barring Service check is required on this application, I agree to allow the Vale of Glamorgan Council’s Licensing Section to retain a copy of my Disclosure and Barring Service Certificate in my Hackney Carriage/Private Hire Driver’s file.
I understand that if I fail to provide the Vale of Glamorgan Council’s Licensing Section with a Disclosure and Barring Service Certificate within six weeks of the date of this application my Hackney Carriage/ Private Hire Driver’s Licences may be revoked with immediate effect.

(1)All applicants for driver’s licences are required to submit a Disclosure Barring Service (DBS) check in respect of their first application and every three years thereafter.

(2)Produce a medical certificate (required if applying for the first time or if renewing your licence after your 45th, 50th, 55th, 60thor 65thbirthday, then annually after age 65) signed by your own registered general practitioner to the effect that you are physically fit to be a driver of a Hackney Carriage or Private Hire vehicle.

(3)Produce your driving licence for inspection. NO TAXI DRIVER LICENCE CAN BE GRANTED UNLESS A DVLA DRIVING LICENCE OR PROOF THAT YOU HOLD A CURRENT LICENCE FROM THE DVLA IS PRODUCED.
