Form 30 Instructions

General: Submit a separate form for each PERP-registered equipment unit. Submit this form by March 1st of each year for the previous year’s operation. Report amount of material processed in each air district where the equipment was operated.

This form is for reporting operation of equipment units, not engines. There are no reporting requirements for engines.

  1. Company Name - Legal name of entity, business, organization, agency or private individual that operates the registered portable engine or equipment unit.
  2. PERP Registration Number - Enter the registration number from the registration certificate or sticker.
  3. Reporting Year – Enter the year for which the equipment unit operation is being reported.
  4. Total Material Throughput – Enter the total annual material throughput for equipment units. Use the units as listed in the Operating Conditions or the units listed below for the type of equipment.
