Welcome/Introductions: Kris Sherrill of Southern Illinois Healthcare welcomed everyone to the meeting and introductions were given of all in attendance.

Announcements/Agency Information Sharing/Handouts:
§ Web-based E-Referral Manual – For Jackson County and the surrounding region has been updated and is located on the SIH Website. This web-based referral manual has information regarding primary care, dental care, mental health services, financial support, housing assistance, wellness and linkage resources, etc. Most resources link directly to an agency’s website so you can be sure you are receiving the most up-to-date information. Go to: www.sih.net then click on “Community Benefits,” then go to Jackson County Healthy Communities Coalition and look under “Member Tools.”
§ The Summit on Aging, Education & Service – September 26, 2011 @ 9:30 AM – 4 PM at Illinois State University Alumni Center, 1101 N. Main St., Normal, IL. The Illinois Policy Academy on the Civic Engagement of Older Adults, and the Intergenerational Initiative, Center for Health Law and Policy, SIU School of Law invite all to attend this workshop/discussion on how to tap the knowledge and skills of all generations to increase student achievement, specifically reading and writing. The goal is an Eight-Point Action Plan for Illinois. Registration is $15 and includes lunch and snack. § Palliative Care: The Nurse’s Role – September 16, 2011 @ 8 AM – 12:30 PM at the SIH System Office in conference room 101 A-D. This workshop is to enhance the care provider’s skills and awareness regarding the care of patient’s through the stages of chronic or life-limiting illness. § Poison Prevention Educator Training – October 4, 2011 @ 1 – 3 PM at the SIH System Office in conference room 101 A&C. By registering you will receive the necessary materials to act as a Poison Prevention Educator for your area as well as your agency. It also qualifies you to receive free poison prevention education materials directly from the Illinois Poison Center (IPC) website. § Professionals Interacting with Older Adults Seminar – September 9, 2011 at 8:30 – 3:30 PM. (Registration starts at 8 AM) at John A. Logan College. This seminar is for professional caregivers or someone who works with caregivers. This one day seminar will discuss topics to help professional care givers in their interactions with seniors in their professional life. Action Team Updates:
§ Healthy Lifestyles
Ø Healthy Living Guide – The action team is in the process of proofing the guide. If anyone has any changes, please contact Kris Sherrill.
Ø START! Walking Paths – still looking for other locations. Working on a draft on-line Start Walking Path submission form. When finalized, it will be distributed to the action team and placed on the community health calendar.
Ø Diabetes Support Group – Meets the last Tuesday of most months at 6 p.m. at Franklin Hospital. Contact Sheila English of Bi-County Health for exact dates at (618) 439-0951.
Ø McDonald’s Press Conference – On Wednesday, September 7, 2011 at 9 a.m. there will be a press conference at the Murdale McDonald’s (2101 West Main) in Carbondale. The press conference will kick off and discuss the Healthy Happy Meals Project (phase 2) in Southern Illinois, which was developed by Short Enterprises and the Healthy Southern Illinois Delta Network.
Ø Community Leaders Forum – In the process of getting decision makers together for a forum to be held on October 27, 2011. A select group of individuals will be invited to attend to learn how policy, systems, and environmental changes can impact our community’s health.
Ø Caleb Nehring has agreed to be the new co-chair of the Healthy Lifestyles Action Team.

§ Access To Care
Ø Kris Sherrill informed the committee that the newly combined group of the F/WHCC and JCHCC Access To Care Action Team has been meeting the last couple of months and concentrating their efforts on Oral Health Care issues (fluoride/varnishing treatments as well as an enhanced referral program).
Ø This group has also created a E-resource guide and would like to expand it to cover all 3 counties. Currently, the resource guide is accessible on the SIH website at www.sih.net

F/WHCC Logo: Kris Sherrill reminded everyone that when using the F/WHCC logo to remember to keep the integrity of the logo intact. When resizing, please remember to pull logo from the corners rather from the top or bottom.
Community Health Calendar: Kris Sherrill asked everyone present to promote the Community health Calendar website by linking it from their website, etc. Magnets are also available for distribution. F/WHCC Meeting Locations/Dates: Nancy Muzzarelli announced that the Man-Tra-Con office located at the VF Factory Outlet Mall in West Frankfort has closed so the October 20th meeting will now be held at the West Frankfort Aquatics Center. A discussion was also held concerning future locations of the 2012 meetings. It was agreed by all present that holding the meetings at the Marion Man-Tra-Con was the preference rather than trying to find a new location in West Frankfort.

Agency Presentation: Whitney Mehaffy and Carrie Eldridge of Franklin-Williamson Bi-County Health Department gave this month’s agency presentation on the IPLAN Community Needs Assessment.

Definition of a health problem: A situation or condition of people which is considered undesirable, likely to exist in the future, and measured as death, disease, or disability. It was noted that community members are needed in order to give input on top health problems in Franklin and Williamson counties. An on-line survey link is available on the Franklin/Williamson Bi-County Health Department website www.bicountyhealth.org for community members to take on the leading health problems in the area. Paper surveys were also distributed for anyone interested in distributing to their clients. The deadline for these surveys are September 16, 2012.
