What is the All about me & All about us document?
The all about me/us builds Section A of your final Education, Health and Care plan (EHCP) if one is required. This should be completed by both you and your parents/carers. Direct quotes from young people themselves should be indicated by encasing them in speech marks. Schools can help you and your parents/carers complete these sections or Independent Supporters are available to help too. Schools may already have developed a person centred approach to reviews in which case other ways of recording your views such as a recent One Page Profile can be attached instead. The working well and not working well sections will also help develop the outcomes for the plan at your transfer review.

How do I fill it in?

Below we have put together some questions underneath each of the All About Me/ Us sections which you may want to consider when filling in the section.

  • What is important to you in school
  • How you communicate

How to support me?
Tell us how it is best to support you for example:
  • Things that make life easier for you
  • How to keep you safe
  • How to help you to learn and communicate.
  • Any medicine you need to take on a regular basis and how you take this medication
  • Any particular equipment you use and why.
  • What support you may need to make decisions.eg interpreter, signer, supportive friend or adult

It is important to make plans for the future and things you would like to do. This could include:
  • Short term plans
  • Longer term plans
  • Further education aspirations – at the end of year 11 do you want to stay at your current school or go to college or get an apprenticeship/traineeship or supported internship
  • Employment ideas – what job or career you are thinking about
  • Your current wishes for future independence– longer term would you like to live on your own, with friends, in supported accommodation or stay with your family
  • Other things you would like to be able to do

Preferences for your next learning provider
Please let us know where you would like to continue learning at the end of Year 11, or if appropriate at the end of Year 13 or Year 14
The options open to you will include
  • To stay at your current school in the sixth form, if they have one
  • Go to another school sixth form
  • Go to a further education college
  • Go to a sixth form college
  • Start an apprenticeship or traineeship
  • Start a supported internship
  • Go to an independent specialist provider. This option would only be considered by the local authority if local providers are unable to meet your needs.
  • You have the opportunity to name your preference for the next learning provider in the final EHCP – you can’t though name a course. You need to take into account that some courses may require specific GCSE passes as entry requirement, so it is important that you have a “back up plan”

This section is also an opportunity to talk about what is important to you when you are working with people who support your child.Do you need to have appointment dates set for certain days or timesand
are there certain topics that should not be discussed in front of some members of the family?

  • Is there is a service that you are accessing that has had a particularly positive impact on your family life?

Things that are not working so well for our family
Think about your family’s life at the moment. Tell us what is not working well and why?In education, with healthcare or with keeping safe how could things be improved?
You may want to consider things such as:
  • specific difficulties e.g. transport arrangement that are not suitable, or a
group that is available but not easily accessed.
You may also like to consider more complex issues such as;
  • a family member that may need further support in coming to terms with your family’s circumstances / child’s diagnosis;
  • full time job / family responsibilities and managing your child’s needs
  • personal relationships and how your child’s care needs have / are impacting upon these.
  • the impact upon your / your family’s health and wellbeing
  • your ability to seek employment or do leisure activities.
