Judge Name of the Judge of the Name of the County County Juvenile Court committed your child to the Department of Juvenile Justice for “care, supervision, and planning”. A screening committee meeting will be held at Location of Screening on Date of Screening at Time of Screening. The purpose of the meeting is to determine where Youth's First Name will live while committed to the Department.

You and Youth's First Name have the right to appear before the screening committee and give relevant facts and/or recommendations concerning the placement. Although this is not an adversary proceeding, you may bring an attorney to the screening. You and Youth's First Name also have the right to appeal the recommendation made by the screening committee within 5 working days of the screening. Appeals made after 5 working days will not be considered. Appeals are made by contacting the District Director listed below in writing, by fax, or by email:

Although the Department may legally remove your child from your care for some period of time, you will retain your parental rights and the responsibility for working with the Department in providing for the physical, mental, and emotional health and the moral development of your child. This will require you to participate with the Department in planning for Youth's First Name’s return to your home, visiting Youth's First Name if the placement is in a program, and arranging home visits unless prohibited by a court order. Additionally, if your child is placed in a residential program, you may be asked to send allowance money, money for clothing, and to assist with other incidental expenses as you would if your child were in your own home. If it is not possible for your child to reside with you upon return to the community, it is imperative that you indicate at the screening who you would like the child to live with. You should bring that person(s) to the screening meeting. The Department will take your wishes into consideration, but placement outside of your home will require criminal background checks for the adults in the home.
