Description of Service / 25 bedded unit which provides care for patients who have a Portsmouth GP and are over 18 years of age.
They are admitted for either end of life care of continuing care assessments to see where long term needs can be met. Predominantly a nurse led service

Continuing Care process and assessment
Gold Standard Framework meeting weekly to discuss end of life patients with GP, nurses and SPC.
MDT meetings with Social Worker, Physiotherapist, Occupational Therapist and Nurses to discuss progress and discharge plans
Work/Shift pattern / Over 24 hours
7:30 – 3:00pm 12:00 – 9:00pm or on occasions 7:30 – 9:00pm
Dress/Uniform code / Uniform required
Staff room / Rest Room facilities / On site Microwave available for use and toaster, tea and coffee making facilities
Library / IT Resources / Online via staff intranet
Induction / Orientation Programmes
Welcome Folder / Students are allocated a mentor and a buddy
Student Tutorial / Seminar Programme (if available)
Prior recommended reading / websites / resources / Solent NHS Trust website as a minimum
