HUDSON VALLEY HEALTHCARE ORGANIZATIONS RECEIVE 2013

IPRO QUALITY AWARDS

Recognized for Lowering Medicare Beneficiaries’ Hospital Readmissions

(Lake Success, NY – June 13, 2013) – Twenty-nineHudson Valley healthcare organizations have been named recipients of 2013 IPRO Quality Awards. The awards, given annually by IPRO, recognize organizations and professionals that demonstrate a commitment to improving the quality of care provided to New York’s Medicare beneficiaries. IPRO isthe Medicare Quality Improvement Organization (QIO) for New York State, and manages the state’s End- Stage Renal Disease (ESRD) Network for the Centers for Medicare & Medicaid Services (CMS). The awards were given during IPRO’s 29th Annual Meeting, June 4that the LaGuardia Marriott Hotel.

“These awardees have shown that, by making a major commitment to quality improvement and partnering with other organizations, they can reduce rehospitalizations and improve the communication and care management of patients across the continuum of care,” says Clare B. Bradley MD, MPH, Senior Vice President and Chief Medical Officer, IPRO. “We applaud their achievements.”

As members of the Hudson Valley Care Transitions Coalition, these organizations have demonstrated organization-wide commitment to improving transitions of care for Medicare beneficiaries through community partnerships. The community coalition’s implementation of evidence-based, cross-setting interventions resulted in a 32% relative improvement in the Medicare fee-for-service, 30-day, all case readmission rate for beneficiaries in the region.

IPRO is convening community coalitions with the goal of activating patients and engaging healthcare providers and community organizations to improve transitional care and reduce unnecessary hospital readmissions. These coalitions support seamless transitions of patients from the hospital to home, skilled nursing care, home health care or hospice through process improvements that address medication management, post-discharge follow-up, and patient care plans.

A study published recently in the Journal of the American Medical Association (JAMA) showed that communities that worked with IPRO and other QIOsin 2009 and 2010 on improving transitions of care had nearly twice the reduction in hospitalizations and rehospitalizations as those not working with QIOs. QIOs are independent organizations that contract with the CMS to work collaboratively with healthcare providers and professionals to help improve care for Medicare beneficiaries. IPRO worked on this project with providers in New York’s Upper Capital region during this time.
