**Fees include a full uniform for each registered player.

Medical: Please list any medical condition and/or physical limitations, (i.e. Asthma, Diabetes, etc.) your child may have. Your signature below acknowledges that in the absence of a parent or guardian, a Manager, Coach, Officer, or Board member of the Bauerstown Baseball and Softball Association may obtain medical treatment for your child in the event of serious injury or illness.

The coaches select teams. Special requests cannot be guaranteed. The BBSA Executive Board has the right to review and decide on special requests based on circumstance. All fees paid are final and not subject to refunds. Late signups cannot be guaranteed placement on a team.

I hereby agree to indemnify and hold harmless the Bauerstown Baseball and Softball Association, including any of its coaches or officials, for any injury to my child, even if caused by negligence of player, coach, or official. I further understand that there is limited insurance other than my own.

As a member of Shaler Area Baseball Association (SABA) and the Greater Pittsburgh Girls Softball League (GPGSL), Bauerstown Baseball And Softball Association has agreed to follow their by-laws. By registering with BBSA, I also agree to follow SABA, GPGSL, and BBSA by-laws.

Registration Dates

January 7th and 8th

and

January 14th and 15th

Place: Cherry City Fire Hall

Time: 10:00 am to 2:00 pm

Sell your tickets to recoup the $50 family fee!

Raffle tickets will not be available after the above dates.

If you are unable to attend Registration,

please mail your registration form with your check to:

All registrations must be received by March 1st or be subject to a $1000 late fee.

SAMPLES OF ALL UNIFORM SIZES WILL BE AVAILABLE AT REGISTRATION.

We are not able to reorder uniforms because of incorrectly ordered sizes.
