I ______(print or type your name), am a senior of the Maryland School System, and I’m applying for The Bryan Michael Burnett Scholarship because I am planning to attend an institution of higher learning to pursue a career in Culinary Arts or Hospitality Management. I do hereby agree that any/all information provided on this application is true to the best of my knowledge. I understand that should any part of this application be falsified, I will no longer be considered for the scholarship, and will be prohibited from reapplying. I must verify enrollment in College/Trade School as part of my application process. I understand that the total amount of the award is $500.00,and will be disbursed at the end of the first semester upon receipt of verification of grades by the Foundation (grades to be sent within four weeks of issuance). Grades received passed the four week period will not be acknowledged. The applicant must provide proof of a cumulative grade point average (GPA) of 2.5 or better before funds will bedisbursed. I understand if the required GPA is not met I will be disqualified to receive the disbursement of the scholarship fund. I also do hereby understand that should I withdraw from school before completing the first semester, there will be no funds disbursed.

SCHOLARSHIP APPLICATION FORM
The Bryan Michael Burnett Scholarship Fund is awarded to any Maryland High School graduating senior who plan to attend an institute of higher learning. To allow other students an opportunity to apply for the scholarship, this is a one-time only award of $500.00. Funds will be disbursed to the appropriate institution after verification of the applicant’s semester grade point average has been received by the scholarship review committee.
The criteria are as follows:
- The student must be a Maryland High School Graduate
- Minimum grade point average of 2.5
- Two (2) letters of recommendations from the applicant’s instructor/ counselors/administrators/pastor/community leader. The letters must be received by the committee in a sealed envelope from the persons completing the recommendation, with name address, and telephone number (to be verified).
- A 300-word essay that includes the following:
- Your future plans
- Why you are applying for the scholarship?
- Why you feel you would be the best candidate to receive the scholarship from the Bryan Michael Burnett scholarship Foundation, Inc.
- How will the scholarship fund assist you in obtaining your goals?
- Submit copies of all awards/certificates while enrolled in high school.
- Verify enrollment/acceptance to an institution of higher learning.
- Verify Grade Point Average after completion of first (1st) semester at the chosen institution of higher learning.
- All applications (with completed information) must be submitted no later than April 15th of each year. Applications postmarked after April 15th will not be returned or considered. Funds will be disbursed to the applicant’s chosen institution once the scholarship review committee has made their decision and the applicant has been notified. Under No Circumstances Will Funds Be Disbursed To The Students.
