Job Title: Careers & Transitions Coordinator

Job Location Percy Hedley College

Hours: Full Time

Salary: Point 19-26 - £17,980 - £ 22,443

Annual Leave: 30 days plus bank holidays and statutory days

Line Managed by: Curriculum Manager

Responsible to: Vice Principal

Job Purpose:

·  Provide an effective Careers education programme for PHC students

·  To coordinate College careers & transition processes providing regular & timely information to CMT as required

·  To develop and maintain the extensive networks, links and relationships with the agencies and providers involved in Careers & Transition of College students

·  To target, liaise and network with an effective range of external organisations and professionals associated with young people with disabilities, to ensure effective independent advice & guidance for transition

·  To keep abreast of developments within Local Authority relating to education provision, day services/personal budgets and have an effective working knowledge of the College funding methodology.

Connexions/IAG

·  To support the Curriculum Manager with the College’s contract/ contacts with the local Connexions Services.

·  To provide students access to advice and guidance as requested by the Curriculum Manager

·  To maintain the Connexions library with up to date information on career and residential information for students.

·  To deliver transition support to college students as required by the Curriculum Manager by regular timetabled sessions

Transition: Leavers

·  To contribute to the process of Annual Review meetings, liaising with the Vice Principal as required

·  To chair Annual Reviews of final year students and in liaison with CMT

·  To take the lead on co-ordination of student transition out of College, to ensure maximum options and effective arrangements are in place for leavers.

·  To liaise with Personal Tutors and with external professionals involved such as Connexions Advisers, Social Workers and Transition Managers.

·  To arrange regular Transition meetings with Personal Tutors and therapy staff to provide advice, guidance and support relating to the transition from College.

·  To report on/contribute to Transition meetings.

·  To complete Destination records as required by the Curriculum Manager

·  To maintain Destination data on Databridge in liaison the CMT

·  To maintain an effective knowledge of personal budgets/LA funding & post college options

·  To organise & attend external visits as requested by the Curriculum Manager

Other duties

·  To be a member of the Operational Management Team preparing reports as requested

·  To maintain records of statistical data relating to Transition and liaise with College Managers and Administrators.

·  To work closely with the Recruitment & Assessment coordinator
