Doctor of Pharmacy

Title:

Clinical pharmacist, Pharm.D.

Job Summary

Supervise and coordinate the delivery of pharmacy related services in the clinic.

Serve as the pharmacy department’s representative to administration and medical staff.

Provide direct patient care, Medication Therapy Management (MTM) services

Qualifications

  1. Completion of Doctor of Pharmacy Program from an accredited university
  1. Licensure as a pharmacist with the Minnesota Board of Pharmacy
  1. Completion of an ASHP accredited residency program
Responsibilities
  1. Director of Pharmacy Services
  2. Responsible for operations related to medication delivery or distribution to patients and departments within the organization
  3. Direct contact with administration in order to allow for safe and accurate delivery of pharmacy services within the system
  1. Clinic

Consult with patients/families/providers regarding drug therapy management or monitoring. The clinic visit with patients includes the following components:

  • Obtain necessary patient history in order to provide safe and effective medication management
  • Perform focused physical examination
  • Analyze patient information and laboratory data for all aspects of medical therapy
  • Analyze factors that interfere with safe/effective medication use
  • Formulate plan of care obtaining additional lab studies, adjust medication dosages, arrange follow-up visits, provide appropriate patient education and provide intervention or referral for newly identified problems within current scope of practice
  • Discuss treatment plans with other health care providers, as necessary
  • Provide education for patients and families about medication use
  • Provide self-management education and training in chronic disease states
  • Provide appropriate phone follow-up, when necessary, on clinical issues
  • Fully document all clinical services in patient chart

Clinic Administrative

  • Supervise all pharmacy students that have a clinical rotation in the clinic
  • Work with clinic management to assure appropriate reimbursement for clinical services
  • Evaluate current clinical services through QA/I
  • Investigate and implement additional services
  • Keep Scope of Practice up-to-date with current practice
  • Work with clinic nurse manager on medication stock and nursing/pharmacy issues
  1. Patient and Provider Medication Education
  2. Provide education for patients, families, and providers regarding disease state pathophysiology and treatment, medication risks, benefits, and precautions.
  3. Provide medication information to medical staff as needed
  1. University of MN College of Pharmacy Clinical Faculty
  • Serve as preceptor to Pharm.D. students. Supervise student activities and provide on-site education for students. Support student professional and patient projects.
  1. Diabetes Self Management Team
  2. Educate patients with diabetes and their families in accordance with national ADA standards.
  3. As requested, periodically educate medical staff on ADA Standards of Medical Care and work to increase compliance with those standards.

  2. Prepare formal educational materials and documents as they relate to job aspects listed herein
  3. Review medical texts and literature as it relates to job aspects listed herein

Physical Involvement

Must be able to sit, bend, turn, and maintain other related body postures frequently to perform a majority of the job duties.

Must be able to sit and/or stand frequently for periods during an 8 hour work day

Must be able to visually utilize computers and other office equipment, to properly enter information, as well as read required documentation.

Must be able to make precise visual inspections, as well as read detailed specifications, instructions, and medication orders for extended periods of time

Must be able to lift/carry up to 20 pounds, occasionally, an average distance of from 5 to 20 feet,

Must be able to tolerate the frequent grasping and fine finger motions.

Must be able to perform repetitive hand and arm motions.

Must be able to read required documentation.

Must be able to prepare concise, accurate written documents.

Must be able to communicate clearly with all levels of management and employees.

Mental Involvement

Must possess the ability to plan, organize and delegate responsibilities to subordinates while maintaining quality standards throughout one’s own performance.

Excellent communication skills are required for interactions with management, staff, patients, residents, clients, etc..

Must be able to analyze situations and select a course of action.

Must be capable of multi-step and sequential problem solving activities; that include comparing, analyzing and calculating data relevant to the facility.

Must be flexible and willing to undertake a variety of tasks.

Must possess the ability to accept change and respond appropriately.

Must possess the ability to understand and communicate both verbally and in written form in the English language since instructions, labels and other documents are in English.

Must possess visual and mental attention to position content with accuracy.

Must possess excellent memory and organizational skills.

Must be able to plan, organize and perform a sequence of operations where standard methods are available.

WORKING CONDITIONS

Work is primarily performed within a clinic setting.

Temperature and humidity are regulated while indoors.

Proper lifting techniques and caution in handling materials must be used to ensure the safety of the employee.
