Employee Handbook Checklist
Please complete this checklist and return it to us either by email, fax or hard copy. We’ll develop your employee handbook and have a draft in your hands in a matter of weeks. In addition, we’ll include a variety of other important, “tried and true” policies so that you will have a comprehensive, up to date employee handbook. If you don’t have one of the policies listed below, just make a few notes of points in which you are interested and we’ll develop the policy for you. If there is a special policy for your organization that you would like to have included, just let us know and we’ll make sure it’s a part of your handbook.
- History of the Employer – include either a previously written history or bullet points and we’ll write it for you.
- Introductory Period (sometimes called Probationary or Try Out Period).
- Notice Required on resignations – management and non-management.
- Policy on employee having another job (sometimes called “moonlighting”).
- Do you conduct exit interviews?
- When do compensation reviews occur? Annually? Periodically? No policy?
- When do you conduct performance appraisals? Annually? Periodically? No policy?
- What is your normal workweek (day to day)?
- When is payday?
- What is your overtime policy?
- What is your method of time keeping? Which employees are required to keep time?
- What is your vacation policy?
- What paid holidays do you observe?
- What is your sick leave policy?
- Do you have a personal leave policy? If so, please describe.
- Are you covered by Family and Medical Leave (you must have 50 employees within 75 miles)?
- What insurance do you provide to your employees and what portion does the employer pay?
- What is your meal period policy?
- What is your policy on employee work breaks?
- Do you provide educational benefits to your employees? If so please describe.
- What is your dress code policy (include tattoos, piercings, fragrances, extreme styles, etc.)? Do you require some employees to wear uniforms? If so, which ones? Does the employer pay any of the costs associated with uniforms?
- Do you have a policy allowing for employees to purchase goods or services at a discount?
- What is your smoking policy?
- Do you provide a retirement plan of any kind? 401(k)? Profit sharing? Pension? If so, please describe briefly (your handbook will contain a brief policy but will not take the place of your Summary Plan Description). Does the employer contribute? If so, how much?
- What is your parking policy, if any?
- Do any employees drive employer owned vehicles, or drive personal vehicles on employer business? If so, what is the maintenance policy? The policy on employee accidents? May the employee take the employer owned vehicle home after work?
- Does the employer require or provide health examinations or tests? If so, please describe.
- What is your policy on cell phones/mobile phones/smart phones at work?
- For your sexual harassment policy – to whom would you like employees to report complaints of sexual harassment (should have two lines of reporting).
- What is your policy on drugs and alcohol at work?
- What is your policy on weapons in the workplace?
- Do you have an employee search policy?
- What is your policy on Zero Tolerance for Violence at work?
- Are employees allowed to date or have a social relationship with other employees? With managers or supervisors?
- What is your policy on absenteeism and lateness?
- Do you have published rules and regulations?
- What is your policy on employment of relatives?
- What is your policy on employees having community involvement, such asvoting and serving on the jury?
- Are there any special employee benefits that we have not asked about in this checklist?
- Do you have any additional policies that you would like for us to include?
