APPLICATION FOR MEMBERSHIP

______applies for membership in the Virginia Youth Soccer Association, Inc.

1. Provide the names, addresses, phone numbers (home and office), and e-mail addresses for all officials of your organization.

2. List the number of teams your organization has by age group and gender (all male, all female, co-ed).

3. State your principal geographical areas and seasons of play.

4. Identify any sponsorship of your organization, including governmental entities.

5. Attach a copy of each of the following from your organization:
articles of incorporation, constitution, bylaws, playing rules and regulations (must include small sided play for younger age groups).

6. Provide the name, address, phone numbers (home and office), and e-mail address of one individual to whom all VYSA mailings should be sent. Also include your club office address, phone, e-mail and web site if applicable.

7. Attach the annual player registration fees for the seasonal year September 1 through the following August 31 in the amount of:

Recreational Club Players:

$9.00 times the number of players

Travel League Players:

$13.00 times the number of players

8. ______agrees to abide by the following:

(1)  to provide information about players, coaches, other team officials, and its officials

as required by the VYSA Board of Directors, US Youth Soccer, and US Soccer;

(2) to obtain insurance through VYSA or provide evidence of insurance as required by
the VYSA Board of Directors;

A completed application will be considered and voted on at the next meeting of the Board of Directors. The affirmative votes of a majority of the Board members is required for an applicant to become a member.

CURRENT INSURANCE REQUIREMENTS

VYSA requires that you obtain insurance through VYSA or that you have purchased insurance directly on your own.

If you have your own insurance, you must provide VYSA with a certificate of insurance from your insurance company showing that you have at least $1,000, 000 injury liability insurance. You must provide this proof of insurance within 30 days after being accepted for membership in VYSA and thereafter within the first 30 days of each renewal period.

TIMELY SUBMISSION OF APPLICATION

To insure adequate time to review the information submitted with your membership application and the completeness of that information, as well as any questions that may need to be answered, please submit your application for membership at least 30 days before the VYSA Board of Directors meeting which you would like your application considered and approved. Failure to do so may result in a delay in the Board’s consideration of your application until its next meeting
