Provide the following notes in the pertinent section of the plans as necessary.

FAA DETERMINATION

Designers may refer to “SDDOT Airport Coordination and Clearance Process for Construction Projects” found in Chapter 16 of the Road Design Manual.

Designers must review the FAA Determinations for their project provided by the SDDOT Office of Aeronautics to obtain study expiration dates and required notes.

SDDOT has notified FAA of the proposed construction or alteration associated with this project. The filing with FAA included notification of list the temporary equipment/structure and permanent structure and corresponding locations by table or note here.

The Contractor must abide by the following FAA temporary structure (crane) determination requirements as set forth in FAA DeterminationsNo. 2017-AGL-21583-OE, 2017-AGL-21584-OE, 2017-AGL-21585-OE, and 2017-AGL-21586-OE.

As a condition of this determination, the temporary crane must be lowered to the ground when not in use and during the hours between sunset and sunrise. During daylight hours the crane must display either a 3-foot by 3-foot square flag having a checkered pattern of international orange and white squares at least 1 foot on each side that is on a staff attached to the crane so that the flag will be readily visible or a yellow flashing light that is mounted on the uppermost part of the crane structure. If the crane is erected between sunset and sunrise then it must be lighted in accordance with FAA Advisory Circular 70/7460-1 L Change 1, Obstruction Marking and Lighting, Chapters 4, 5(Red), and 12. This information is in the FAA Determinations or it could be as recommended by the Office of Aeronautics.

The crane height must not exceed 80 feet above ground(1380 feet above mean sea level). This information is in the FAA Determination.

AERONAUTICAL NOTIFICATION (TEMPORARY STRUCTURE: DEFINE STRUCTURE)

It is required that the manager of the XXX Airport (605) ###-#### be notified by the Contractor at least 3 business days prior to the temporary structure (crane) being erected and again when the structure is removed from the site. The advanced notice is required to allow the airport manager to file a Notice to Airmen NOTAM which alerts pilots to the presence of the crane. This information is either located in the FAA Determination or it could be as recommended by the Office of Aeronautics.

The Contractor will notify Engineer at the South Dakota Department of Transportation XXX Area Office (605) ###-####by Date if a crane remains at the project site. The SDDOT will file for a determination extension with the FAA.

The South Dakota Department of Transportation is responsible for e-filing the FDC Notice to Airmen 3 business days prior to the temporary crane being erected and again within 3 days of it being removed from the site. The Contractor shall notify Robert Ward at the South Dakota Department of Transportation Aberdeen Area Office (605) 626-3307 and provide the following NOTAM information:

1. The date the crane will be erected.

2. Provide the date when the crane will be removed.

3. Provide a time of arrival.

4. Provide an onsite Contact Name and Cell Number.

The above information is in the FAA Determination.

AERONAUTICAL NOTIFICATION (PERMANENT STRUCTURE TYPE(S))

The South Dakota Department of Transportation has received FAA Determinations for the permanent structures as listed in the FAA Determination plan note.

The Contractor shall notify Engineering Supervisor at the SDDOT XXXArea Office (605) ###-####if alterations are proposed for the heights or coordinates of a luminaire, bridge, box culvert. The SDDOT will re-file for an updated FAA determination. The Contractor will not be allowed to proceed with construction of the proposed altered installation until notified by the SDDOT that FAA approval has been received.

The SDDOT is responsible for filing Part 2 of Federal Aviation Administration Form 7460-2, Notice of Actual Construction or Alteration within 5 days after the construction of the Bridge or Luminaire reaches its greatest height. The Contractor shall notify Engineer at the SDDOTXXXArea Office (605) ###-#### within 24 hours after this has occurred to ensure filing is completed by the FAA deadline.

The Contractor shall notify Engineer at the SDDOT XXXArea Office (605) ###-#### by Date if the construction of the structure has not reached its greatest height. The SDDOT will file the FAA required determination extension.
