Please complete the forms in this packet.

Youth Chorus

Completed forms must be returned when you audition 6/2 or 6/4, 6:00 pm at the Miami Valley Centre Mall. Enter between Elder-Beerman and Sears.

Senior Chorus

Completed forms must be returned when you audition for:

Female Solo, 6/3, 7:00 pm

Male Solo, 6/5, 7:00 pm

Senior Chorus, 6/2 or 6/4, 7:00 pm

If you are 18 years or older, you can sign your own forms.

· All members must have white shorts, white shoes.

· Participation fee for the Youth Chorus is $15.00

· Participation fee for the Senior Chorus is $25.00

· Report any and all absences to Pam Crabtree, 778-1578 (home) 773-8550 (work), or e-mail

All auditions and rehearsals are at the Miami Valley Centre Mall. All admittance is thru the rear entrance.

Contract of Rules and Regulations

1. Attendance: I will see to it that I attend all rehearsals in which I am included. I will let the staff know in advance of all absences for vacation, camps, etc. I have marked my calendar with my absences. More than five (5) absences, excused or unexcused, will result in dismissal. I will make every attempt not to use all five (5) absences. No last minute surprises! Music Warehouse Board of Directors will have the final say on dismissals.

2. Attitude: I will come to practice to have a good time and work hard toward our goals. I will not have an attitude or spread my bad mood around to others.

3. Respect: I will respect all Music Warehouse staff members. This includes directors, dance captains, student leaders, parents, band members, sound and lights, and any adults helping with the show.

4. Final Week: The final week is crucial!! Attendance is near mandatory for the final week of rehearsal.

− Music Warehouse is not a school activity

− Music Warehouse is not for credit

− Music Warehouse is volunteer

− Music Warehouse is a commitment

− Music Warehouse is a blast!

Release

Recognizing the possibility of physical injury associated with dancing and otherwise performing upon a musical production stage and/or raised platform erected upon such stage, and for Music Warehouse's accepting the Performer for its practices, performances and other activities, I hereby release, discharge, and/or otherwise indemnify Music Warehouse, its affiliated organizations, sponsors, members, Trustees, Directors, Officers, committee members, agents, and employees, for and against any claim by or on behalf of the Performer as a result of the Performer's participation in Music Warehouse.

As a Music Warehouse participant, I hereby release the use of photographs, video, and audio of my child or me for publicity and/or public use.

The purpose of this form is to enable parents or guardians to authorize or refuse the provision of emergency treatment for children who become ill or are injured while under Music Warehouse authority when parents or guardians cannot be reached.

Part I To Grant Consent:

In the event reasonable attempts to contact my designee or me have been unsuccessful, I hereby give my consent for the administration of any treatment deemed necessary by family physician or dentist. In the event the designated practitioner is not available, authorization is granted for treatment by another licensed physician or dentist. This authorization does not cover major surgery unless the medical opinions of two other licensed physicians or dentists, concurring in the necessity for such surgery are obtained prior to the performance of such surgery.
