It is my/our desire to have my/our child/children enrolled in the daycare program at Westside Learning Academy. I/we have received a copy of the Westside Learning Academy policy handbook. I/we have read, understand and agree to abide by the policies contained therein. I/we also understand that my/our child will be assessed throughout the school year and I/we give my/our permission to do so. I/we further understand that if the policies outlined in this handbook were not adhered to, it would be sufficient cause for the removal of the child/children from the program. I/we also agree to give a minimum of two weeks written notice (ten full preschool days) of my/our intent to withdraw my/our child/children from the preschool/extended day program. If two weeks notice is not given, I/we agree to make full tuition payment for the final two weeks.
The word “Center” as used in this agreement, shall be deemed to refer to Westside Learning Academy.

______I/we understand that I/we must provide a completed medical form to the preschool.
______I/we understand the Center tuition is due Monday. Late fees are $10 a week.
______I/we have contracted for the hours of ______to ______.
______I/we understand that we are required to pay for the entire week, regardless of attendance.
______I/we understand that we DO NOT pay for the weeks that the Center is closed (Thanksgiving
Break, Christmas Break, Spring Break)
______I/we understand the late pickup/early drop off fee is $1.00 per minute.
______I/we understand the pickup policy for other than parental pick up.
______I/we understand the illness policy.
______I/we understand the meal policy.
______I/we are contracting for (school year only, summer only) arrangements.
______I/we understand the behavior policy and I/we have read and shared the Center rules with my/our
children.
______I/we understand the returned check policy.
______I/we understand that I/we must notify the Center immediately of any phone number, change of
address or any other enrollment information changes for either parent/guardian.
______I/we understand that I/we will be responsible for all bank and collection charges plus legal fees
and disbursements on a solicitor and his own client basis incurred by the Center in the collection
of fees or the enforcement of any of the terms of this agreement.
______I/we understand that the Center shall not be responsible for any loss or damage to the clothing or
the property of the child while such child is in attendance at the Center or participating in its programs.
______I/we understand that we must notify in writing of any special requirements (including food, life
threatening allergic and other dietary restrictions) arising from ethnic or religious customs to prevent infringement due to lack of such knowledge.
