SUBJECT: Revised Administrative Order/Notice of Monthly Support Obligation (Form CS-66) and Procedural Instructions, Notification of Review Determination (Form CS-79) and Procedural instructions, Rights and Responsibilities of Noncustodial Parents (Form CS-80) and Procedural Instructions, Health Insurance Information Request (Form CS-136) and Procedural Instructions.
Forms CS-66, CS-79, CS-80, CS-136, and their Procedural Instructions have been revised to reflect the changes made to the related regulation 921 KAR 1:400 and to format the header and footer to comply with the Department’s forms protocol. The CS-66, CS-79, CS-80, CS-136 and their Procedural Instructions have been updated on the CHFS Intranet Forms page. The Spanish versions of the revised CS-66, CS-79, CS-80, and CS-136 will be added to the Intranet once they have been translated by the Cabinet for Health and Family Services Language Access Section.
The revisions, as specified by form number, are as follows:
· The CS-66 has been revised to clarify when the noncustodial parent and the custodial parent are to receive the Administrative Notice of Monthly Support Obligation (Form CS-66). Under the general procedures section, the Procedural Instructions have been revised to delete the wording, “after the CS-66 becomes final and effective” for an initial CS-66. This will allow both parties to receive the notice within the same timeframe. The timeframe of 20 days to request an administrative hearing on an initial Notice of monthly support Obligation has been included for commonality between the Procedural Instructions and the CS-66. Item 12 of the form has been amended to state that either parent can object and request an administrative hearing within 20 days of receipt of the notice for an initial notice and within 30 days of receipt of the notice for a modified notice.
· The CS-79 has been revised to incorporate the form into Regulation 921 KAR 1:400. The Procedural Instructions for the CS-79 have been revised to include the fourteen (14) day timeframe that the caseworker has to send the CS-79 to the custodial parent and noncustodial parent.
· The CS-80 has been revised to remove the list of specific enforcement actions that may be taken if the total amount of support owed is not paid. The Procedural Instructions were revised to reflect the name change to Administrative Hearings and the Kentucky Child Support Program (PAM CS-82).

· The CS-136 has been revised to rephrase the statement concerning whether the child(ren) are covered by health insurance. Page two (2) of the form has been amended to request any health insurance policy information that covers the child(ren) listed on page one (1), not just a policy of the custodial or noncustodial parent. The Procedural Instructions for the CS-136 were revised to remind workers that the form may be printed from the Division of Child Support website and manually completed if necessary.
This transmittal letter is to be saved and filed for future reference as it explains changes that have been made to the Kentucky Child Support Forms. The Kentucky Child Support Forms are to be updated as shown below.
