This service which provides forensic, medical and aftercare services to survivors of rape/sexual assault who either self refer or are referred via the police or other agencies. The postholder will undertake sexual offence examiner clinical duties, which will include: providing an immediate response to Police or self referrals; providing a forensic medical examination after a reported recent sexual assault; providing written reports and advice to Police Officers; as well as preparing witness statements for use in Court and attending Court to give objective unbiased evidence. You will also be able to develop skills and experience as a Specialty Doctor in integrated Sexual and Reproductive Health Services based at Sandyford Central in Glasgow and throughout NHS Greater Glasgow and Clyde.

All applicants must have Full GMC Registration, a licence to practice and at least 4 Years full time Post Graduate Training, two of which must have been within a Specialty Training programme or you will demonstrate and evidence equivalent experience and competencies.

JOB SUMMARY

Postholder will undertake clinical duties within Archway Glasgow sexual assault service based at Sandyford and within Sandyford’s integrated sexual health service.

Archway Glasgow provides forensic, medical and aftercare services to survivors of rape/sexual assault who self refer or are referred via the police or other agencies.

Duties include providing an immediate response to Police or self referrals providing a forensic medical examination after a reported recent sexual assault, providing written reports and advice to Police Officers as well as preparing witness statements for use in Court and attending Court to give objective unbiased evidence.

There will be a formal training and induction programme and requirement to demonstrate core competencies to the lead clinician prior to commencing on call rota duties. The successful applicant will also be required to attend 50% of the monthly peer review and development sessions. Further training will be provided as necessary in addition to clinical supervision from the lead Consultant for the Archway Glasgow.

The postholder will also work within Sandyford Central integrated sexual health services, including provision of all forms of contraception, testing and treatment for a range of sexual infections, and the opportunity to develop skills in medical gynaecology, termination of pregnancy and referral services, and a range of other specialist services.

Location:Based at Sandyford, Glasgow (In emergencies, it maybe necessary to conduct examinations elsewhere, for example at an Accident & Emergency Department or within an operating theatre of an acute hospital).

2.Entry Requirements

Candidates must be fully registered with the GMC.

A minimum of four years full time postgraduate general professional training is requiredat least two of which will be in a specialty training programme in a relevant specialty.

A current DFSRH or MFSRH or eligibility to apply for these qualifications as well as previous experience of work in Genitourinary Medicine, General Practice, Obstetrics & Gynaecology or forensic medicine is desirable.

3.Sandyford

Sandyford is a comprehensive specialist service for sexual, reproductive and emotional health for NHS Greater Glasgow and Clyde, hosted within NHS Greater Glasgow and Clyde North West CHP. Sandyford works closely with Health Board colleagues, Universities and other bodies within the health board area and covers a population of 1.1 million stretching from East Glasgow to Inverclyde.

Sandyford ethos is ‘to provide health improving services in a client focused, inclusive and anti-discriminatory way, through constantly innovating to meet the needs of men and women’. The aim of the Initiative is to promote excellence in sexual and reproductive health, using the Sandyford ethos, and to influence others to provide positive, open access approaches to health and well-being for all.

Archway Glasgow

Glasgow Archway supports those who have been raped or sexually assaulted by providing forensic and medical services, counselling and follow-on support for women, men and young people who have been raped or sexually assaulted.Its secondary aim is to effectively and efficiently manage cases from disclosure to potential conviction. It is the first service of its kind in Scotland and of high significance as a potential model for other future developments. Currently, the service covers people raped or sexually assaulted in the former geographical boundary of Strathclyde Police area now Police Scotland

Archway Glasgow is an NHS service within NHSGGC. It isfunded by NHSGGC, Police Scotland and Glasgow City Council

Glasgow Archway is located within the Sandyford, Greater Glasgow and Clyde’s city-wide sexual, reproductive and emotional health service and works with partner agencies, specifically Police Scotland , to support those who have been raped and sexually assaulted, as well as to inform wider social and political agendas.

Archway Glasgow operates within the context of the UN definition of male violence that has been adopted by the Glasgow Violence Against Women Partnership. It operates a co-ordinated approach, based on respect for dignity and integrity of the survivor as an individual that is responsive to the whole person and their specific needs throughout the process of disclosure reporting, investigation, prosecution and recovery following rape and sexual assault.

Archway Glasgow staff recognise the social context within which rape and sexual assault occurs and the need for their practise to be informed by awareness and sensitivity of social and cultural factors which shape the lives of those using the service. By its very nature, Archway Glasgow is a highly sensitive service that requires staff confidentiality at all times.

Archway Glasgowhas a core staff team comprising a lead Consultant,2 full-time Nurses, Manager and Administrator.There are also 2 support workers and counsellors available during normal working hours.

In addition, there is a pool of on call Doctors – Sexual Offences Examiners (SOEs) and supporton call Nurses who provide services on an on-call basis covering 7 days a week of rota cover.

4.Archway Duties and Responsibilities

The SOE is assisted by a support nurse and will be responsible for addressing the immediate considerations of the survivor. This includes the following:

  • Taking a full relevant history
  • Crisis support
  • Survivor safety
  • Forensic examination
  • Securing forensic evidence
  • Corroboration of evidence
  • Providing formal statement to Police
  • Assessing and responding to medical and psychological sequelae
  • Facilitating access to follow-on services
  • Provision of written and oral evidence as required by the Crown Office and Procurator Fiscal Service (COPFS) and other statutory agencies, e.g. Criminal Injuries Compensation Authority

The postholder will also be required to participate fully in meetings and training as necessary and to work as part of a multi-disciplinary team, following protocols and procedures of the Archway Glasgow and Sandyford. They will liaise with the Police, Procurator Fiscal and paediatric and general practitioner services as necessary.

The postholder will be required to follow appropriate training to undertake paediatric examinations of children (aged 13-16 yrs) where acute sexual assault is suspected.

The postholder will be required to liaise with the Archway Glasgow Lead Paediatrician with regard to Archway Glasgow clients aged 13-15.

Main Tasks

Forensic Medical Examination and administration

To provide an immediate response to Police or self referrals requiring a forensic medical examination after a rape or sexual assault. Examinations will usually take place at the Archway Glasgow, Sandyford, but may, in an emergency, be conducted elsewhere such as the hospital Accident & Emergency Department.

Provide appropriate medical care including screening arrangements for sexually transmitted infections, emergency contraception (hormonal & IUD) and advice to individuals disclosing sexual violence.

Accurately to record details of examinations and provide standardised written reports/advice to the Procurator Fiscal Service and the Archway Glasgow staff. The content and quality of notes will be reviewed & overseen by the Lead Nurse and Consultants.

To provide witness statements as appropriate.

To attend Court and give evidence.

To complete detailed forensic medical notes using Archway Glasgow pro-forma which maybe disclosed to Court at the completion of each individual examination. Return to the unit may be necessary after call out to correct notes.

To provide Archway Glasgow staff with necessary details required for the computer database.

To prepare medical reports to the statutory authorities such as the Criminal Injuries Compensation Authority.

Development & Training

To attend compulsory special training in the medical investigation and care of adults and children of serious sexual assault. This will be provided free of charge and includes basic forensic training as well as further training in the examination of children and court training. The postholder will also undertake peer review and be supervised on a one to one basis by the lead Consultant or equivalent for the Archway Glasgow. Training will also be undertaken as required around the service ethos and the social model of health.

To attend initial training covering relevant topics including forensic assessment of child victims, child protection, court training, diversity training and health and safety. Additional peer support, training and review will be offered on a monthly basis and minimal acceptable attendance rate would be 6 sessions a year (50% attendance).

To participate in monitoring and evaluation of the work of the Archway Glasgow and be involved in regular review and case audit in additional to keeping a confidential case log.

To keep up to date about current developments and research about sexual assault and related areas.

To attend a best SARC practice course approved by the FFLM every 3 years

Aftercare/Follow up

The postholder will be responsible for providing emergency contraception to women and appropriate care for men, women and young people including screening and management of sexually transmitted infections.

The postholder will normally be expected to undertake one sexual/reproductive health clinic within the Sandyford service at a minimum of once a fortnight depending on experience in the field of sexual and reproductive health.

The postholder will be expected to adhere to all Archway Glasgow clinical and administrative aftercare protocols and guidelines.

The postholder would be encouraged to consider working towards qualifications in this field such as DFCASA (diploma of forensic ans clinical aspects of sexual assault ) or MFFLM (Membership of the Faculty of Forensic and Legal Medicine)

5. Sandyford integrated sexual health services duties

  • Provide direct clinical input within integrated Sexual & Reproductive Health and related services at Sandyford or any of its other sites. Flexibility to travel across the geographical spread of NHSGG&C is desirable.
  • To provide and take part in educational activities within and for Sandyford services.
  • Undertake administrative tasks in order to ensure smooth liaison and partnership development with all relevant statutory and non-statutory agencies.
  • Any other task deemed suitable that may arise as a result at the request of management.
  • Participate in Clinical Governance, Appraisal, local and national Audit, Standard Setting, Research and Quality Control as appropriate.

6.Conditions of Service

The postholder will be employed by Sandyford which is part of Greater Glasgow and Clyde NHS. Attendance at the Health Board and medical examination maybe required prior to confirmation of appointment. The postholder maybe required to provide DNA for the DNA Elimination Database locally. Conditions of service for hospital medical staff as defined by the General Whitley Council conditions will apply where appropriate.

Hours of Work And Additional Duty Hours

Individuals are employed under the terms and conditions of a Specialty doctor with an agreed rate of pay based on the Specialty Doctor scale. Where on that scale individuals are appointed depends on qualifications and experience.

On call payments are based on point 6 of that scale pro rata, currently £56,884 per annum.

For post holders employed within Archway and Sandyford.

The weekday session(s) will be agreed with the post-holder to suit the needs of the service. Weekday hours of Archway Glasgow operation are typically between the hours of8am to 5pm. and for Sandyford Services 8am to 8pm.

Based on a full-time, 10 professional activity (PA) post, it is likely that 4 PAs will be within Archway, 4 PAs within Sandyford sexual health services, and 2 PAs for SPA time, audit activity and non-patient facing direct clinical care.

For each 24 hour period of on call in a calendar month individuals will paid 2 pas plus 11.59% (WTR) of their basic salary.

In recognition that this oncall rota will be provided out of hours, the 2 PAs including the WTR payment will be paid at Premium rate; i.e. time and a third.

At present payments are calculated based on average callout for the unit to see a new individual presentation over 3 months. The unit would anticipate to see an average of 350 new individuals a year. To date the average number of callouts over 3 months has always been within 0-1.5 – though an individual clinician may be called out more than once in their 24 hr shift.

You may be asked to return to amend or clarify notes within the 24 hour cover.

If an individual doctor is only able to offer 16 hour cover (5.00pm – 9.00am) sessional payment will be made on a pro rata basis

Additional remuneration or time in lieu will be calculated on a pro rata sessional basis for callout to give evidence.

As the callout periods cover 365 days a year, individuals will be expected to take a share of Bank Holiday cover.

8.Other Facilities

  1. Full use of professional support services within Sandyford including the professional and public library and comprehensive range of Post Graduate courses, lectures and meetings will be available.
  1. Metered car parking is easily available during the day at Sandyford. Public transport in Glasgow is generally good. Parking space for night time duties would be provided.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.
