Section 1
1. Is the flight a scheduled operation or,
An unscheduled flight operated between scheduled city pairs in same equipment or,
A reroute of a scheduled flight to an airport or alternate airport listed in StarTrac Yes No
(If no, proceed to Section 2)
2. Is the departure, enroute, and arrival weather adequate for the flight? Yes No
3. Does the aircraft have any limitations that will affect the flight? Yes No
4. Does the aircraft have the minimum dispatch fuel required for the flight? Yes No
If all highlighted answers are checked authorization can be given.
Section 2 (Unscheduled Flights other than ferry flights or maintenance test flights)
1. Is the airport listed in [company data base as suitable for aircraft type? Yes No
(If yes, proceed to question 7. If no, answer Q2 – 6)
2. Is runway length greater than 6000 feet? Yes No
3. Is the airport elevation less than 2000 feet? Yes No
4. Is there a tower in operation? Yes No
5. Is there official reported weather? Yes No
6. Is there an ILS or VOR approach available? Yes No
(If Q2-7 are all yes, continue to Q8. If any are no, refer to the operational control chart for flight release)
7. Are you an [upper level person with flight release authority] or above? Yes No
(If yes, answers Q8-10. If no, go to Section 3)
8. Is the departure, enroute, and arrival weather adequate for the flight? Yes No
9. Does the aircraft have any limitations that will affect the flight? Yes No
10. Does the aircraft have the minimum dispatch fuel required for the flight? Yes No
If all highlighted answers are checked authorization can be given
Section 3 (Additional requirements for [persons without upper-level flight release authority)
1. Is the departure and arrival weather VFR? Yes No
(If no, refer to the operational control chart for flight release)
2. Is there any significant weather enroute? Yes No
3. Does the aircraft have any limitations that will affect the flight? Yes No
4. Does the aircraft have the minimum dispatch fuel required for the flight? Yes No
If all highlighted answers are checked, flight can be released; if not, upper management must be consulted.
