VERIFIED PETITION FOR ORDER AUTHORIZING PAYMENT OF GUARDIAN’S FEES AND COSTS

COMES NOW the Petitioner, , and alleges as follows:

  1. is duly appointed and acting guardian for , incapacitated having been appointed on , as guardian of the .
  2. Petitioner is a professional guardian and has been since (month and year) and is seeking an hourly rate of $.
  3. The Petitioner had rendered services as the Guardian of the above named incapacitated person from to for which the Petitioner has received no compensation.
  4. Petitioner’s records indicate that during the period of time above mentioned hours were devoted to the duties of Guardian of the above named incapacitated person.
  5. Itemized on the forms attached hereto, and by reference made a part hereof, are the dates and times in increments of 1/10th of an hour by activity code, expended on behalf of the above named incapacitated person, the sum total of which is hours and minutes.
  6. The Petitioner is of the opinion that a reasonable fee for the services performed by the Petitioner during the said period is $, which is calculated at $ per hour.
  7. The criteria of Florida Statute 744.108(2), which the Court shall consider, are more fully described as an attached exhibit.
