From November 19, 2012 through December 21, 2012, Edwardsville Glen Carbon Chamber of Commerce will promote, by means of Web, print, word-of-mouth, social media, media outlets and at participating businesses and sweepstakes drawing locations the opportunity to submit entries to win four (4) weekly prizes and one (1) grand prize as detailed below. Void outside of Illinois.
The contest will be conducted November 19, 2012 through December 21, 2012. Deadline for entries is December 21, 2012 at 11:59 a.m. CST. Four (4) weekly prize drawings will be held at 1:00 p.m. at the Edwardsville/Glen Carbon Chamber of Commerce office on the following dates: November 30, 2012; December 7, 2012; December 14, 2012; and December 21, 2012. The Grand Prize drawing will be held at the Edwardsville/Glen Carbon Chamber of Commerce office at 1:15pm December 21, 2012. The Edwardsville/Glen Carbon Chamber of Commerce office is located at 200 University Park Drive, Suite 260, Edwardsville, IL 62025.
ELIGIBILITY:
You must be 18 years of age or older to win. No purchase necessary. A purchase does not increase chances of winning. One entry per person per visit. Multiple entries per person will be allowed and there is no residency requirement.
The contest is open to everyone with the exception of Edwardsville Glen Carbon Chamber of Commerce Board of Directors, employees and their families, agents and representatives. “Immediate family members” shall mean parents, stepparents, children, stepchildren, siblings, stepsiblings, or spouses. “Household members” shall mean people who share the same residence at least three months in one calendar year.
HOW TO ENTER:
Use a smart phone to scan the QR code, which is prominently featured on all signage. The QR code will connect users to an online entry form they may complete using a smart phone; OR go to www.edglenchamber.com and click the ‘Shop Chamber Campaign Entry Form’ button. Complete online entry form at www.edglenchamber.com on or before December 21, 2012 at 11:59 a.m. CST; OR for manual entry come to Ed/Glen Chamber office to complete a paper form or include requested information by Edwardsville/Glen Carbon Chamber of Commerce on a plain 8½” by 11” piece of paper.
Manual entries must delivered to the Ed/Glen Chamber office between the hours of 9 a.m.-4 p.m. CST or may be mailed be mailed to:

All sweepstakes entries become the property of Edwardsville/Glen Carbon Chamber of Commerce and will not be returned and will not be used by Edwardsville/Glen Carbon Chamber of Commerce for any purpose and in any media in perpetuity.
Names and phone numbers will not be shared or used for any form of solicitation.
Entries will accumulate and qualify for four (4) Weekly Drawings and one (1) Grand Prize drawing. Odds of winning are dependent upon the number of eligible entries received for the drawing.
DESCRIPTION OF FOUR (4) WEEKLY PRIZES:
Four (4) Weekly Prizes are a gift of certificates for goods and services. Value will vary in the range of $150 to $400.
Weekly Prize winners must pick up the prize at the Edwardsville/Glen Carbon Chamber of Commerce office, located at 200 University Park Drive, Suite 260, Edwardsville, IL 62025, or make alternate arrangements within 72 hours of notification.
The odds of winning depend on the number of eligible entries received. The winner will be awarded a gift of certificates for goods and services valued in the range of $150 to $400.
Prizes are not transferrable. All taxes, shipping, licenses, deliver, fees, and charges associated with prizes are solely the winner’s responsibility. All prize details (including without limitation, method and location of delivery of the prize) are at sponsor’s sole discretion.
DESCRIPTION OF ONE (1) GRAND PRIZE:
One (1) Grand Prize is $1,000 cash.
Grand Prize winner must pick up the prize at the Edwardsville/Glen Carbon Chamber of Commerce office, located at 200 University Park Drive, Suite 260, Edwardsville, IL 62025, or make alternate arrangements within 72 hours of notification.
Edwardsville/Glen Carbon Chamber of Commerce will choose one winner from all eligible entries received at random. The odds of winning depend on the number of eligible entries received. The winner will be awarded a gift of $1,000 cash.
Prize is not transferrable. All taxes, shipping, licenses, deliver, fees, and charges associated with prizes are solely the winner’s responsibility. All prize details (including without limitation, method and location of delivery of the prize) are at sponsor’s sole discretion.
HOW TO WIN:
A total of four (4) Weekly Drawing winners and one (1) Grand Prize winner will be selected in a random drawing conducted from all qualified entries at dates and locations listed above.
In the event that the original winner does not meet Official Rules or eligibility requirements, or he/she or parent or legal guardian forfeits the prize, Edwardsville/Glen Carbon Chamber of Commerce will award the prize to the first alternate. Process will continue until prize is awarded.
Winner will be notified by phone number provided on ticket and will be required to sign and return an Affidavit of Eligibility and Publicity/Liability Release form within 72 hours of notification in order to claim the prize.
Prize will not be released until Edwardsville/Glen Carbon Chamber of Commerce receives all required documents. Failure of winner to return the forms within the required timeframe will result in automatic forfeiture of any prizes and all such prizes shall be awarded to an alternate winner.
UNFORSEEN EVENTS:
Edwardsville Glen Carbon Chamber of Commerce reserves the right to change this sweepstakes at its sole discretion at any time that Edwardsville Glen Carbon Chamber of Commerce becomes aware of any circumstances or occurrence which would materially affect the sweepstakes security or fairness. Edwardsville Glen Carbon Chamber of Commerce reserves the right to announce any sweepstakes rules changes over the air, in print or on posters. Edwardsville Glen Carbon Chamber of Commerce is not responsible for the inability of the winner to accept the prize for any reason.
PUBLICITY:
Winner will be required to sign and return a release of liability, affidavit of eligibility, and a publicity consent agreement, all within 72 hours of acknowledged notification. By accepting and/or using prize, winner agrees to the use of their names, quotes, and photos for the purpose of advertising, trade, or promotion without further compensation. If a selected winner cannot be contacted, is ineligible, fails to claim a prize, or fails to timely return the completed and executed affidavit and releases as required, prize will be forfeited and an alternate winner will be selected.
CONDITIONS:
By entering, participants agree to be bound by these Official Rules and the decisions of Edwardsville Glen Carbon Chamber of Commerce and the sponsors, which shall be final. Sponsors, agents and representatives of Sponsors, their parent companies, affiliates, subsidiaries, advertising, promotion, and fulfillment agencies, and legal advisors, are not responsible for and shall not be liable for:
· late, lost, delayed, damaged, misdirected, incomplete, illegible, unintelligible, or postage-due entries;
· telephone, electronic, hardware or software program, network, Internet, or computer malfunctions, failures, or difficulties of any kind;
· failed, incomplete, garbled, or delayed computer transmissions;
· any condition caused by events beyond the control of the Sponsors that may cause the Sweepstakes to be disrupted or corrupted;
· any injuries, losses, or damages of any kind arising in connection with or as a result of the prize, or acceptance, possession, or use of the prize, or from participation in the Sweepstakes; or
· any printing or typographical errors in any materials associated with the Sweepstakes. Edwardsville Glen Carbon Chamber of Commerce reserves the right, in its sole discretion, to suspend or cancel this Sweepstakes at any time if the administration, security, or proper play of the Sweepstakes is corrupted at any time.
TAXES:
All taxes are winner's responsibility and a Form 1099 will be issued to the Grand Prize winner. All taxes relative to the prize package are the sole responsibility of the winner. No prize substitutions, assignments or transfers. No cash equivalents. The reporting and payment of all local, state and federal taxes incurred by accepting a prize will be the sole responsibility of the prize winner.

DISPUTES:
Entrants agree that any and all claims, disputes and causes of action arising out of or related to the Sweepstakes shall be resolved, without resort to any form of class action, exclusively by the appropriate courts in Madison County, Illinois in accordance with the laws of the State of Illinois, without giving effect to conflict of law rules which would cause the application of the laws of any other jurisdiction.
Sweepstakes Entities shall not be responsible for technical, pictorial, typographical or editorial errors or omissions in any marketing materials or these Official Rules.
All decisions are final. Sponsor makes no warranty, representation or guarantee, express or implied, in fact or in law, relative to the use of the Sweepstakes prize including, but not limited to, quality, merchantability or fitness for a particular prize.
