Dear prospective participant/vendor:
It’s that time of year; time to start preparing for the Annual Christmas Boutique at the First Foursquare Church, which will take place Saturday, November 21, 2009.
All proceeds made by our Annual Christmas Boutique go towards sending youth grades 3rd – 12th to summer camp in Sonora, CA at Old Oak Ranch. Each year youth earn the opportunity to attend a week long summer camp that has various activities such as church services, swimming, hiking, basketball, volleyball, paintball, archery, a ropes course and many other activities. We thank you for your participation!
All vendors are encouraged to call or email the event coordinator to make sure that there are not duplicate vendors. Please note that we will only allow 1 vendor per company represented and 3 vendors for every type of handmade merchandise (example: only 1 Cookie Lee vendor and up to 3 vendors that hand make jewelry).
All applications and fees must be postmarked by November 7, 2009 and mailed to address listed.

RELEASE OF LIABILITY
In consideration of such participation, participant agrees to assume all risk (including but not limited to weather, sales, merchandise, etc…) from such participation and agrees to hold the First Foursquare Church and its staff and volunteers harmless from all suites, claims or demands of every kind. Participant agrees that submitted application and fees are NON-REFUNDABLE. If for any reason the application is not accepted the original payment will be mailed back to prospective participant at address listed above.
